摘 要:目的总结电刺激(EStim)技术治疗骨骼肌肉系统疾病的临床效果及其作用机制方面的研究进展。方法在中国知网、万方数据、Springer Link、PubMed等中、英文数据库中,检索自2000年1月以后公开发表的有关EStim与骨骼肌肉系统的文章共2778篇,最终纳入57篇文献进行系统复习及归纳总结。结果EStim技术对骨骼、肌肉、肌腱、韧带、关节软骨等骨骼肌肉系统各组成部分的疾病,如骨折、骨质疏松、肌肉萎缩,以及肌腱、韧带、关节软骨和周围神经损伤等,均有积极的治疗效果,且具备经济性、安全性、便捷性与有效性等优势。EStim对骨骼肌肉系统疾病的作用机制是通过物理、分子生物学等途径,对骨骼肌肉系统的血液循环、细胞分化、机械运动、蛋白质代谢等产生影响,甚至有可能促进软骨细胞增殖、加速神经轴突再生。结论采用EStim技术治疗骨骼肌肉系统疾病,其方法可行、疗效肯定;其作用机制是通过物理和生化两个方面的途径来实现的。Objective This paper amid to summarize the progress of research on the clinical effectiveness and mechanism of electrical stimulation(EStim)for the treatment of musculoskeletal system diseases.Methods Published articles on EStim and musculoskeletal system since January 2000 were searched in CNKI,Wanfang Database,Springer Link,PubMed,and other Chinese and English databases.A total of 2778 articles were searched,among which 57 were finally included after screening the summary.Results EStim technology had positive therapeutic benefits for diseases of various components of the musculoskeletal system,such as bone,muscle,tendon,ligament and articular cartilage.The diseases included fracture,osteoporosis and muscle atrophy,as well as tendon,ligament,articular cartilage and peripheral nerve injury.Given its advantages of economy,safety,convenience,and effectiveness,the therapy had ideal clinical application value.The mechanism of EStim's action on musculoskeletal system diseases involved physical and molecular biological pathways.EStim affected local blood circulation,cell differentiation,mechanical movement,and protein metabolism of musculoskeletal system.It may even promote the proliferation of chondrocytes and accelerate the regeneration of nerve axons.Conclusion EStim technique is feasible and effective for the treatment of musculoskeletal system diseases.It has ideal clinical application prospects.The mechanism of its action is through physical and biochemical approaches.
