阴道用乳杆菌活菌胶囊序贯抗生素治疗混合性阴道炎患者的效果

摘　　要：目的:观察阴道用乳杆菌活菌胶囊序贯抗生素治疗混合性阴道炎患者的效果。方法:选取2022年2月至2023年2月该院收治的120例混合性阴道炎患者进行前瞻性研究,按照随机数字表法将其分为对照组与观察组各60例。对照组采用阴道用乳杆菌活菌胶囊同步联合抗生素(甲硝唑阴道泡腾片、2%克林霉素磷酸酯阴道凝胶)治疗,观察组在抗生素停药3 d后,序贯使用阴道用乳杆菌活菌胶囊治疗。比较两组临床疗效,临床症状(外阴瘙痒、阴道烧灼感、分泌物增多)消失时间,治疗前后阴道微生态指标(pH值、菌群多样性、菌群密集度、优势菌)、血清氧化应激指标[丙二醛(MDA)、超氧化物歧化酶(SOD)、内皮素1(ET-1)]水平,复发率,以及不良反应发生率。结果:观察组治疗总有效率为96.67%(58/60),高于对照组的81.67%(49/60),差异有统计学意义(P<0.05);观察组外阴瘙痒、阴道灼烧感、分泌物增多等症状消失时间均短于对照组,差异有统计学意义(P<0.05);治疗后,观察组阴道pH值、菌群多样性、菌群密集度、优势菌正常率及血清SOD水平均高于对照组,血清MDA、ET-1水平均低于对照组,差异有统计学意义(P<0.05);治疗3个月后,观察组复发率为3.33%(2/60),低于对照组的13.33%(8/60),差异有统计学意义(P<0.05)。两组均未发生严重不良反应。结论:与同步用药相比,阴道用乳杆菌活菌胶囊序贯抗生素治疗混合性阴道炎患者的效果显著,可加快临床症状消失,促进阴道微生态环境恢复,减轻氧化应激反应,并降低复发率。Objective:To observe effects of live Lactobacillus capsule for vaginal use sequential antibiotics in treatment of patients with mixed vaginitis.Methods:A prospective study was conducted on 120 patients with mixed vaginitis admitted to the hospital from February 2022to February 2023.According to the random number table method,they were divided into control group and observation group,60 cases in each group.The control group was treated with live Lactobacillus capsule for vaginal use combined with antibiotics(Metronidazole vaginal effervescent tablets,2% Clindamycin phosphate vaginal gel),while the observation group was treated with live Lactobacillus capsule for vaginal use sequential antibiotics treatment after 3 days of antibiotic withdrawal.The clinical efficacy,the disappearance time of clinical symptoms(vulva pruritus,vaginal burning sensation,increased secretions),the vaginal microecological indexes(pH value,flora diversity,flora density,dominant bacteria),and the serum oxidative stress indexes [malondialdehyde(MDA),superoxide dismutase(SOD),endothelin(ET-1)] levels before and after the treatment,the recurrence rate,and the incidence of adverse reactions were compared between the two groups.Results:The total effective rate of the observation group was 96.67%(58/60),which was higher than 81.67%(49/60) of the control group,and the difference was statistically significant(P<0.05).The disappearance time of vulva pruritus,vaginal burning sensation and increased secretion in the observation group were shorter than those in the control group,and the differences were statistically significant(P<0.05).After the treatment,the levels of vaginal pH value,flora diversity,flora density,normal rate of dominant bacteria and the serum SOD level in the observation group were higher than those in the control group,while the serum MDA and ET-1 levels were lower than those in the control group,and the differences were statistically significant(P<0.05).After 3 months of treatment,the recurrence rate of the observation group was 3
