复发性流产患者子宫蜕膜血管的免疫病理观察

摘　　要：目的:分析复发性流产患者子宫蜕膜血管的免疫病理情况。方法:选取2021年6月—2022年6月泰安市妇幼保健院收治的复发性流产患者40例作为研究组,选取同期收治的40例疑似复发性流产的既往流产孕妇作为对照组。两组均进行免疫组织化学染色,比较两组蜕膜血管免疫球蛋白G(IgG)、免疫球蛋白M(IgM)、补体C_(3)沉着情况;检测并比较两组血清IgG、IgM、补体C_(3)水平。结果:研究组蜕膜血管IgG沉着率低于对照组,IgM、补体C_(3)沉着率高于对照组,差异有统计学意义(P<0.001)。研究组IgG、IgM水平高于对照组,补体C_(3)水平低于对照组,差异有统计学意义(P<0.001)。结论:子宫蜕膜血管免疫损伤可能是促使复发性流产发生的原因,蜕膜细胞与蜕膜腺上皮细胞均可能存在潜在免疫功能,但其具体机制应进行进一步基础实验探索,这对明确复发性流产发生的原因具有重要意义。Objective:To analyze the immunopathogenesis of uterine decidual vessels in patients with recurrent abortion.Methods:From June 2021 to June 2022,40 patients with recurrent abortion admitted to Taian Maternity and Child Health Hospital were selected as the study group.A total of 40 pregnant women with previous abortion and suspected recurrent abortion admitted during the same period were selected as the control group.Both group underwent immunohistochemical staining.The immunoglobulin G(IgG),immunoglobulin M(IgM)and complement C_(3)deposition in decidual vessels were compared between the two groups.The serum levels of IgG,IgM and complement C_(3)were detected and compared between the two groups.Results:The decidual vascular IgG deposition rate in the study group was lower than that in the control group,and the IgM and complement C_(3)deposition rates in the study group were higher than those in the control group,and the difference was statistically significant(P<0.001).The levels of IgG and IgM in the study group were higher than those in the control group,and the level of complement C_(3)in the study group was lower than that in the control group,and the difference was statistically significant(P<0.001).Conclusion:Uterine decidual vascular immune injury may cause recurrent abortion,and both decidual cells and decidual gland epithelial cells may have potential immune functions,but the specific mechanism should be explored in further basic experiments,which is of great significance for clarifying the cause of recurrent abortion.
