摘 要:目的分析糖尿病肾病(DN)患者血清长链非编码RNA KCNQ1反向链/反义转录物1(LncRNA KCNQ1OT1)、微小核糖核酸-192-5p(miR-192-5p)与白蛋白尿短期进展及预后的关系。方法选取2021年4月—2022年12月康复大学青岛中心医院/青岛市中心医院内分泌风湿免疫科收治的DN患者102例为观察组,根据2次检测(间隔6个月)24 h尿白蛋白及SCr水平分为未进展亚组(n=78)和进展亚组(n=24),根据随访1年预后情况分为预后良好亚组(n=84)和预后不良亚组(n=18),选择同期医院健康体检者58例为健康对照组。采用qRT-PCR检测血清LncRNA KCNQ1OT1、miR-192-5p相对表达量;Pearson法分析LncRNA KCNQ1OT1与miR-192-5p的相关性;多因素Logistic回归分析患者白蛋白尿短期进展的影响因素;绘制受试者工作特征(ROC)曲线分析血清LncRNA KCNQ1OT1,miR-192-5p对患者预后的预测价值。结果与健康对照组比较,观察组血清LncRNA KCNQ1OT1水平升高,miR-192-5p水平降低(t/P=17.619/<0.001、16.120/<0.001);与未进展亚组比较,进展亚组血清LncRNA KCNQ1OT1水平升高,miR-192-5p水平降低(t/P=7.698/<0.001、9.485/<0.001);与未进展亚组比较,进展亚组患高血压比例降低及FPG、HbA_(1c)、BUN、SCr升高,差异均有统计学意义(χ^(2)/P=9.835/0.002,t/P=2.593/0.011、2.356/0.020、4.582/<0.001、3.139/0.002);与预后良好亚组比较,预后不良亚组血清LncRNA KCNQ1OT1水平升高,miR-192-5p水平降低(t/P=7.602/<0.001,9.380/<0.001);LncRNA KCNQ1OT1与miR-192-5p呈负相关(r/P=-0.452/<0.001);高血压、血清LncRNA KCNQ1OT1水平升高为患者白蛋白尿短期进展的危险因素[OR(95%CI)=1.532(1.058~2.219)、1.638(1.100~2.438)],miR-192-5p水平升高为患者白蛋白尿短期进展的保护因素[OR(95%CI)=0.671(0.517~0.871)];LncRNA KCNQ1OT1、miR-192-5p以及二者联合预测患者预后的AUC分别为0.808、0.822、0.896,联合预测显著优于各自单独预测(Z/P=2.030/0.042、2.116/0.034)。结论DN患者血清LncRNA KCNQ1OT1水平升高,miR-192Objective To study and analyze the relationship between serum long non-coding RNA KCNQ1 opposite strand/antisense transcript 1(LncRNA KCNQ1OT1),microribonucleic acid-192-5p(miR-192-5p)and short-term progression of albuminuria and prognosis in patients with diabetic nephropathy.Methods A total of 102 patients with diabetic nephropathy admitted to Department of Endocrine Rheumatology and Immunology,Qingdao Central Hospital,University of Health and Rehabilitation Sciences(Qingdao Central Hospital)from April 2021 to December 2022 were regarded as the disease group.according to the 24 h urine albumin level at time,they were separated into a non-progressing subgroup(n=78)and a progressing subgroup(n=24).According to the prognosis,they were separated into a good prognosis group(n=84)and a poor prognosis group(n=18).58 healthy volunteers who underwent physical examination were regarded as the control group.QRT-PCR was applied to detect the relative expression levels of serum LncRNA KCNQ1OT1 and miR-192-5p.General clinical data of patients were collected and analyzed.Multivariate logistic regression was applied to analyze the influencing factors of short-term progression of albuminuria in patients.Receiver operating characteristic(ROC)curve was plotted to analyze the predictive value of serum LncRNA KCNQ1OT1,miR-192-5p on prognosis.Pearson method was applied to analyze the correlation between LncRNA KCNQ1OT1 and miR-192-5p.Results Compared with the control group,the serum LncRNA KCNQ1OT1 level in the diseased group increased,while the miR-192-5p level decreased(t/P=17.619/<0.001,16.120/<0.001).Compared with the non-progression group,the serum LncRNA KCNQ1OT1 level in the progression group increased,while the miR-192-5p level decreased(t/P=7.698/<0.001,9.485/<0.001).There was a statistically obvious difference in blood urea nitrogen(BUN),creatinine(Scr),fasting blood glucose(FBG),and glycated hemoglobin(HbA_(1c))between the two groups(χ^(2)/P=9.835/0.002,t/P=4.582/<0.001,3.139/0.002,2.593/0.011,2.356/0.020).Compared with
