自拟温阳补土汤治疗脾肾阳虚型肩凝症的临床效果

摘　　要：目的:探讨自拟温阳补土汤治疗脾肾阳虚型肩凝症的临床效果。方法:选取2022年1月—2023年7月阳江市中医医院骨伤科门诊收治的60例脾肾阳虚型肩凝症为研究对象,采用随机数表法分为对照组与治疗组,各30例。对照组采用指导功能锻炼、局部热敷加针灸治疗,治疗组在对照组的基础上加用自拟温阳补土汤治疗。比较两组疗效、治疗前后中医症候积分、肩关节疼痛程度与肩关节功能情况。结果:治疗组疗效总有效率高于对照组,差异有统计学意义(P<0.05);治疗后两组中医症候积分均降低,且治疗组低于对照组,差异有统计学意义(P<0.05);治疗后两组视觉模拟评分法(VAS)评分均降低,且治疗组低于对照组,差异有统计学意义(P<0.05);治疗后两组日本骨科协会肩关节功能量表(JOA)评分均升高,且治疗组高于对照组,差异有统计学意义(P<0.05)。结论:自拟温阳补土汤治疗脾肾阳虚型肩凝症效果确切,能明显改善患者临床症候,减轻肩关节疼痛程度,提升肩关节功能。Objective:To investigated the clinical effect of self-prepared Wenyang Butu Decoction on shoulder coagulation syndrome of spleen-kidney Yang deficiency type.Method:A total of 60 cases of shoulder coagulation syndrome of spleen-kidney Yang deficiency type admitted to the Orthopedics and Traumatology Department of Yangjiang Hospital of Traditional Chinese Medicine from January 2022 to July 2023 were selected and they were divided into control group and treatment group according to the random number table method,30 cases in group.The control group was treated with guided functional exercise,local hot compress combined with acupuncture,and the treatment group was treated with self-prepared Wenyang Butu Decoction on the basis of the control group.The effects,the traditional Chinese medicine syndrome score,the degree of shoulder joint pain and shoulder joint function before and after treatment between the two groups were compared.Result:The total effective rate of the treatment group was higher than that of the control group,the differences were statistically significant(P<0.05).After treatment,the traditional Chinese medicine syndrome scores of the two groups were decreased,and the treatment group was lower than that of the control group,the differences were statistically significant(P<0.05).After treatment,the visual analogue scale(VAS)scores of the two groups were decreased,and the treatment group was lower than that of the control group,the differences were statistically significant(P<0.05).After treatment,the shoulder joint function scale of Japanese Orthopaedic Association(JOA)scores of the two groups were increased,and the treatment group was higher than that of the control group,the differences were statistically significant(P<0.05).Conclusion:Self-prepared Wenyang Butu Decoction is effective in treating shoulder coagulation syndrome of spleen-kidney Yang deficiency type,which can obviously improving the clinical symptoms,reducing the degree of shoulder joint pain and enhancing the shoulder joint function.
