下腔静脉滤器置入联合溶栓治疗对下肢骨折术后同侧下肢深静脉血栓患者的影响

摘　　要：目的:探究下腔静脉滤器置入联合溶栓治疗对下肢骨折术后同侧下肢深静脉血栓患者的影响。方法:回顾性选取2017年8月—2022年8月本院收治的106例下肢骨折术后同侧下肢深静脉血栓患者。根据下肢深静脉血栓不同治疗方式将其分为观察组和常规组,各53例。两组均给予抗凝治疗,常规组给予下腔静脉滤器置入,观察组在常规组基础上给予溶栓治疗。比较两组临床疗效、血栓清除率、治疗前及治疗2周后相关因子、不良反应及滤器取出率。结果:观察组总有效率高于对照组,差异有统计学意义(P<0.05)。观察组血栓清除率为92.45%(49/53),明显高于常规组的71.70%(38/53),差异有统计学意义(P<0.05)。治疗2周后,两组脂蛋白相关磷脂酶A_(2)(LP-PLA_(2))、内皮素-1(ET-1)水平均降低,观察组LP-PLA_(2)、ET-1水平均低于常规组,差异有统计学意义(P<0.05)。观察组滤器取出率为94.34%(50/53),高于常规组的79.25%(42/53),差异有统计学意义(P<0.05)。结论:下腔静脉滤器置入联合溶栓治疗应用于下肢骨折术后同侧下肢深静脉血栓患者,临床疗效显著,可以提高血栓清除率、滤器取出率,降低炎症因子和血管内皮因子,安全性较好。Objective:To explore the effect of inferior vena cava filter placement combined with thrombolysis treatment on patients with ipsilateral lower limb deep vein thrombosis after lower limb fracture.Method:A total of 106 patients with ipsilateral lower limb deep venous thrombosis after lower limb fracture admitted to Hai’an Hospital of Traditional Chinese Medicine from August 2017 to August 2022 was retrospectively selected.According to different treatment methods for lower limb deep vein thrombosis,the patients were divided into observation group and routine group,with 53 cases in each group.Both groups were given anticoagulant therapy,the routine group was given inferior vena cava filter placement,and the observation group was given thrombolysis treatment on the basis of the routine group.The clinical efficacy,thrombus clearance rate,related factors before treatment and after 2 weeks of treatment,adverse reactions and filter removal rate were compared between the two groups.Result:The total effective rate of observation group was higher than that of the routine group,the differences were statistically significant(P<0.05).The thrombus clearance rate in the observation group was 92.45%(49/53),which was significantly higher than 71.70%(38/53)in the routine group,and the difference was statistically significant(P<0.05).After 2 weeks of treatment,the levels of lipoprotein-associated phospholipase A_(2)(LP-PLA_(2)),endothelin-1(ET-1)in both groups were decreased,and the levels of LP-PLA_(2) and ET-1 in the observation group were lower than those in the routine group,the differences were statistically significant(P<0.05).The filter removal rate in observation group was 94.34%(50/53),which was higher than 79.25%(42/53)in routine group,and the difference was statistically significant(P<0.05).Conclusion:The application of inferior vena cava filter implantation combined with thrombolysis treatment in patients with ipsilateral lower limb deep vein thrombosis after lower limb fracture has significant clinical efficacy,which c
