卤米松乳膏联合308 nm准分子激光治疗斑秃的临床疗效

摘　　要：目的 探讨斑秃采用308 nm准分子激光联合卤米松乳膏治疗的临床疗效。方法 回顾性选取斑秃患者100例,依据治疗方法不同分为联合治疗组(308 nm准分子激光联合卤米松乳膏治疗)、单独治疗组(单独卤米松乳膏治疗),各50例。比较两组临床疗效、血清炎症因子[白细胞介素(IL)-6、IL-10、IL-12、白三烯B4(LTB4)、白三烯C4(LTC4)、C反应蛋白(CRP)]水平、不良反应发生情况、复发情况。结果 联合治疗组患者的总有效率92.00%(46/50)高于单独治疗组的60.00%(30/50)(χ^(2)=14.035,P=0.000<0.05)。治疗后,两组患者的血清IL-6、IL-10、IL-12、LTB4、LTC4、CRP水平均低于治疗前,且联合治疗组患者的血清IL-6(0.41±0.12)ng/L、IL-10(2.00±0.35)μg/L、IL-12(63.15±11.15)ng/L、LTB4(2.10±0.31)μg/L、LTC4(2.00±0.36)μg/L、CRP(1.80±0.30)mg/L均低于单独治疗组的(0.46±0.11)ng/L、(2.42±0.45)μg/L、(71.66±15.08)ng/L、(2.55±0.41)μg/L、(2.48±0.40)μg/L、(2.22±0.28)mg/L(P<0.05)。联合治疗组患者的不良反应发生率8.00%(4/50)低于单独治疗组的22.00%(11/50)(χ^(2)=3.843,P=0.049<0.05)。联合治疗组患者的复发率2.00%(1/50)低于单独治疗组的18.00%(9/50)(χ^(2)=7.111,P=0.008<0.05)。结论 斑秃采用308 nm准分子激光联合卤米松乳膏治疗的临床疗效较单独卤米松乳膏治疗显著。Objective To explore the clinical effect of halometasone cream combined with 308 nm excimer laser in treatment of alopecia areata.Methods 100 patients with alopecia areata were retrospectively selected and divided into combined treatment group(308 nm excimer laser combined with halometasone cream treatment)and single treatment group(halometasone cream alone treatment)according to different treatment methods,with 50 cases in each group.The clinical efficacy,serum inflammatory factor[interleukin(IL)-6,IL-10,IL-12,leukotriene B4(LTB4),leukotriene C4(LTC4),C-reactive protein(CRP)]levels,occurrence of adverse reactions and recurrence were compared between the two groups.Results The total effective rate of the combined treatment group was 92.00%(46/50),which was higher than the single treatment group's 60.00%(30/50)(χ^(2)=14.035,P=0.000<0.05).After treatment,the serum levels of IL-6,IL-10,IL-12,LTB4,LTC4 and CRP in both groups were lower than those before treatment;the combined treatment group had IL-6 of(0.41±0.12)ng/L,IL-10 of(2.00±0.35)μg/L,IL-12 of(63.15±11.15)ng/L,LTB4 of(2.10±0.31)μg/L,LTC4 of(2.00±0.36)μg/L and CRP of(1.80±0.30)mg/L,which were lower than(0.46±0.11)ng/L,(2.42±0.45)μg/L,(71.66±15.08)ng/L,(2.55±0.41)μg/L,(2.48±0.40)μg/L and(2.22±0.28)mg/L in the single treatment group(P<0.05).The incidence of adverse reactions in the combined treatment group was 8.00%(4/50),which was lower than the single treatment group's 22.00%(11/50)(χ^(2)=3.843,P=0.049<0.05).The recurrence rate in the combined treatment group was 2.00%(1/50),which was lower than the single treatment group's 18.00%(9/50)(χ^(2)=7.111,P=0.008<0.05).Conclusion The clinical effect of 308 nm excimer laser combined with halometasone cream in the treatment of alopecia areata is more significant than that of halometasone cream alone.
