气压弹道式体外冲击波治疗椎间盘源性下腰痛疗效观察

摘　　要：目的气压弹道式体外冲击波治疗椎间盘源性下腰痛疗效观察。方法选择椎间盘源性下腰痛60例患者。采用随机数字表法分为观察组与对照组,每组各30例。观察组采用压弹道式体外冲击波治疗仪治疗;对照组采用立体动态干扰电治疗。治疗前、后对所有患者进行疼痛视觉模拟评分(VAS)、Oswestry功能障碍指数评分(ODI)和磁共振成像髓核T2弛豫时间值的变化。治疗3个月后,随访两组患者VAS评分和ODI指数变化。结果两组患者治疗前、后VAS、ODI评分比治疗前改善,差异有统计学意义(P<0.05)。观察组治疗后髓核T2值高于对照组,差异有统计学意义(P<0.05)。观察组治疗后3个月VAS评分较治疗后降低,差异有统计学意义(P<0.05)。结论气压弹道式体外冲击波治疗椎间盘源性下腰痛能缓解患者腰部疼痛、改善腰椎功能。Objective To observe the therapeutic effect of pneumatic ballistic extracorporeal shock wave therapy for discogenic lower back pain.Methods 60 patients with discogenic lower back pain were selected.They were divided into an observation group and a control group using a random number table method,with 30 cases in each group.The observation group was treated with a pressure ballistic extracorporeal shock wave therapy device.The control group received three-dimensional dynamic interference electrotherapy.Before and after treatment,all patients were evaluated for changes in pain visual analogue scale(VAS),Oswestry functional impairment index(ODI),and magnetic resonance imaging T2 relaxation time values.After 3 months of treatment,the VAS score and ODI index changes were followed up in both groups of patients.Results The VAS and ODI scores of the two groups of patients improved before and after treatment,and the difference was statistically significant(P<0.05).The T2 value of the nucleus pulposus in the observation group was higher than that in the control group after treatment,and the difference was statistically significant(P<0.05).The VAS score of the observation group decreased significantly after 3 months of treatment compared to after treatment(P<0.05).Conclusion The pneumatic ballistic extracorporeal shock wave therapy for discogenic lower back pain can alleviate lower back pain and improve lumbar function in patients.
