超声检查联合血清糖原合成酶激酶-3β和热休克蛋白27对慢性肺源性心脏病的诊断价值

摘　　要：目的 探讨超声检查联合血清糖原合成酶激酶-3β(GSK-3β)和热休克蛋白27(HSP27对慢性肺源性心脏病(CPHD)的诊断价值。方法 将100例CPHD患者设为观察组,100名健康体检者设为对照组。比较两组受试者一般资料、实验室检查结果、超声检查结果、GSK-3β及HSP27水平。采用Pearson相关性分析法探讨超声检查指标与血清HSP27、GSK-3βmRNA水平的相关性,采用受试者工作特征(ROC)曲线分析超声检查和各项血清指标对CPHD的诊断效能。结果 观察组受试者肺动脉压高于对照组,主肺动脉内径、右室内径及右房内径均长于对照组(P<0.01)。观察组受试者血清HSP27水平高于对照组,血清GSK-3β mRNA水平低于对照组(P<0.01)。血清HSP27水平与肺动脉压、主肺动脉内径、右房内径和右室内径均呈正相关(P<0.05或0.01)。血清GSK-3β mRNA水平与肺动脉压、主肺动脉内径、右房内径和右室内径均呈负相关(P<0.05或0.01)。超声参数、血清GSK-3β、HSP27诊断CPHD的曲线下面积(AUC)分别为0.880、0.834和0.868,联合诊断的AUC为0.923,高于单独诊断。结论 超声参数、血清GSK-3β和HSP27对CPHD均具有较高的诊断价值,联合检测的诊断效果更高。Objective To explore the diagnostic value of ultrasound examination plus glycogen synthase kinase-3β(GSK-3β),and heat shock protein 27(HSP27)in chronic pulmonary heart disease(CPHD).Methods A total of 100 CPHD patients were set as observation group and 100 healthy examinees as controls.Such indexes were compared between two groups as general data,laboratory test results,ultrasonic results,and GSK-3βand HSP27 levels.Pearson correlation analysis was used to explore the correlation of ultrasonic examination indexes with HSP27 and GSK-3βmRNA level,receiver operating characteristic(ROC)curves were used to analyze the diagnostic efficacy of ultrasonic examination and all serum indexes in the CPHD.Results The pulmonary artery pressure was higher and main pulmonary artery diameter,right ventricular diameter and right atrial diameter were all larger in observation than control group(P<0.01).Serum HSP27 level was higher and GSK-3βmRNA lower in observation than control group(P<0.01).Serum HSP27 level was positively correlated with pulmonary artery pressure,main pulmonary artery diameter,right atrial diameter,and right ventricular diameter(P<0.05 or 0.01).Serum GSK-3βmRNA level was negatively correlated with pulmonary artery pressure,main pulmonary artery diameter,right atrial diameter,and right ventricular diameter(P<0.05 or 0.01).The areas under the ROC(AUC)of ultrasound,GSK-3β,HSP27 for the diagnosis of the CPHD were 0.880,0.834 and 0.868 respectively,the AUC of combined diagnosis was 0.923 and higher than that of sep-arate diagnosis.Conclusion Ultrasound,GSK-3βand HSP27 all have high diagnostic value for CPHD and diagnostic efficacy of combined higher.
