放松训练联合呼吸操对急性下肢深静脉血栓患者术前焦虑术后疼痛及治疗配合度的影响

摘　　要：目的 探讨放松训练联合呼吸操对血栓清除术治疗急性下肢深静脉血栓(LEDVT)患者术前焦虑、术后疼痛及治疗配合度的影响。方法 将96例急性LEDVT患者根据随机数字表法分为对照组和观察组,各48例。对照组患者给予常规干预,观察组患者给予放松训练联合呼吸操干预。采用汉密顿焦虑量表(HAMA)、广泛性焦虑量表(GAD-7)、视觉模拟评分法(VAS)、自制治疗配合度调查问卷及纽卡斯尔护理满意度量表(NSNS)评估两组患者术前焦虑情况、术后疼痛、治疗配合度及护理满意度,统计并比较两组患者住院期间并发症发生率。结果 干预后,两组患者HAMA、GAD-7评分均较干预前降低,且观察组患者HAMA、GAD-7、VAS评分均低于对照组(P<0.01);观察组患者治疗配合度、护理总满意度均高于对照组(P<0.05)。观察组患者并发症总发生率低于对照组,但差异无统计学意义(P>0.05)。结论 放松训练联合呼吸操可有效改善LEDVT患者行血栓清除术前的焦虑状态,缓解患者术后疼痛情况,显著提高患者治疗配合度及护理满意度,具有临床推广价值。Objective To explore the effect of relaxation training combined with breathing exercises on preoperative anxiety,postoperative pain and treatment compliance of acute lower extremity deep vein thrombosis(LEDVT)patients undergoing thrombectomy.Methods Ninety-six acute LEDVT patients undergoing thrombectomy were divided into control and observation group according to random number table method,with 48 cases in each.Controls received routine intervention and observation group did relaxation training combined with breathing exercises.The Hamilton Anxiety Scale(HAMA),7-item Generalized Anxiety Disorder Questionnaire(GAD-7),Visual Analogue Scale(VAS),self-designed compliance questionnaire and Newcastle Satisfaction with Nursing Scale(NSNS)were respectively used to evaluate preoperative anxiety,postoperative pain,treatment compliance and nursing satisfaction in both groups,the incidences of complications during hospitalization counted and compared between two groups.Results After intervention the HAMA and GAD-7 scores of both groups decreased and HAMA,GAD-7 and VAS score were lower in observation than control group(P<0.01),treatment compliance and total satisfaction with with nursing were higher in observation than control group(P<0.05).The total incidence of complications was lower in observation than control group,but difference wasn't statistically signif ica nt(P>0.05).C o n c lu s i o n Relaxat ion training combined with breathing exercises could effectively improve preoperative anxiety of acute LEDVT pat ient s undergoing th rombectomy,alleviate postoperative pain,improve treatment compliance and nursing satisfaction significantly,has the value of clinical promotion.
