清热解毒燥湿方熏洗治疗肛周脓肿术后创面临床研究

摘　　要：目的:观察清热解毒燥湿方熏洗治疗肛周脓肿术后创面的临床效果及对炎症因子、相关生长因子的影响。方法:采用随机数字表法将118例肛周脓肿患者分为对照组、治疗组各59例。2组均行低位肛周脓肿一次性根治术治疗,对照组术后给予高锰酸钾坐浴治疗,治疗组给予清热解毒燥湿方熏洗治疗,2组均治疗2周。比较2组术后创面恢复情况及治疗前后中医证候积分、炎症因子、相关生长因子,评估2组临床疗效及不良反应发生情况。结果:治疗后,治疗组总有效率94.92%,高于对照组83.05%(P<0.05);治疗组创面腐肉脱离时间、创面愈合时间均较对照组缩短(P<0.05)。治疗后,2组中医证候积分、白细胞介素-6(IL-6)、C-反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)水平均较对照组降低(P<0.05),成纤维细胞生长因子(FGF)、血管内皮细胞生长因子(VEGF)水平升高(P<0.05),治疗组治疗后中医证候积分、各炎症因子水平低于对照组(P<0.05),各相关生长因子水平高于对照组(P<0.05)。2组均未发生不良反应。结论:清热解毒燥湿方熏洗治疗能够提高肛周脓肿患者术后创面愈合速度,减轻炎症反应,提高相关生长因子水平与治疗效果,安全性较高。Objective:To observe the clinical effect of fumigation and washing with Qingre Jiedu Zaoshi Prescription on postoperative wound of perianal abscess and its effects on inflammatory factors and related growth factors.Methods:A total of 118 cases of patients with perianal abscess were divided into the control group and the treatment group according to the random number table method,with 59 cases in each group.Both groups were given one-time radical resection of low-position perianal abscess;after operation,the control group was treated with sitz bath with potassium permanganate,and the treatment group was treated with Qingre Jiedu Zaoshi Prescription for fumigation and washing.Both groups were treated for two weeks.Traditional Chinese medicine(TCM)syndrome scores,postoperative wound recovery,inflammatory factors,and related growth factors were compared between the two groups before and after treatment;clinical effects and the incidence of adverse reactions were evaluated in the two groups.Results:After treatment,the total effective rate was 94.92%in the treatment group,higher than that of 83.05%in the control group(P<0.05),and the time for slough detachment and wound healing in the treatment group was shorter than that in the control group(P<0.05).After treatment,the TCM syndrome scores,levels of interleukin-6(IL-6),C-reactive protein(CRP),and tumor necrosis factor-α(TNF-α)in the two groups were decreased when compared with those before treatment(P<0.05),and the levels of fibroblast growth factors(FGF)and vascular endothelial growth factor(VEGF)were increased(P<0.05);the TCM syndrome scores level of each inflammatory factor in the treatment group was lower than that in the control group(P<0.05),and the level of each related growth factor in the treatment group was higher than that in the control group(P<0.05).No adverse reactions happened in the two groups.Conclusion:Qingre Jiedu Zaoshi Prescription for fumigation and washing can accelerate the postoperative wound healing of patients with perianal abscess,reduce i
