单磷酸阿糖腺苷联合干扰素对重型手足口病患儿免疫球蛋白水平的影响

摘　　要：目的:探讨单磷酸阿糖腺苷联合干扰素治疗重型手足口病患儿的临床效果及对免疫球蛋白水平的影响,为临床提供参考依据。方法:选取2017年5月—2018年9月于河南科技大学第一附属医院就诊的78例重型手足口病患儿作为研究对象,采用随机数表法将其分为单一组与联合组,每组各39例。单一组给予干扰素治疗,联合组在干扰素治疗的基础上联合单磷酸阿糖腺苷治疗。比较两组患儿症状消失时间、免疫球蛋白水平、炎症因子水平及不良反应发生情况。结果:治疗后,联合组患儿精神恢复、退热、四肢抖动缓解、疱疹消退、神经受累症状消失时间均短于单一组,差异有统计学意义(t=5.692、7.835、9.195、6.391、8.316,P<0.05);治疗结束后5 d,联合组患儿免疫球蛋白M (IgM)、免疫球蛋白G(IgG)、免疫球蛋白A (IgA)水平均高于单一组,差异有统计学意义(t=4.284、12.138、2.124,P<0.05);治疗结束后5 d,联合组患儿肿瘤坏死因子(TNF-α)、白介素-6 (IL-6)、白介素-1β (IL-1β)及白介素-10 (IL-10)均低于单一组,差异有统计学意义(t=19.491、7.799、27.673、11.194,P<0.05)。结论:将单磷酸阿糖腺苷联合干扰素用于重型手足口病患儿治疗能缩短患儿症状改善时间,提高免疫球蛋白水平,降低炎症因子水平,且不良反应发生较少。Objective:To investigate the clinical effects of adenosine monophosphate combined with interferon in children with severe hand,foot and mouth disease and its effect on immunoglobulin levels.Methods:A total of 78 patients with severe HFMD treated from May 2017 to September 2018 were selected and randomly divided into single group and combined group(39 cases in each group).The single group was treated with interferon,and the combined group was treated with vidarabine monophosphate on the basis of the single group.The symptom disappearance time,immunoglobulin level,inflammatory factor level and the occurrence of adverse reactions were compared between the two groups.Results:After treatment,the recovery abatement of fever,limb shaking relief,herpes regression,nerve inoiovement symptom disappearance,and neurological in volvement disappearance time of combined group was shorter than that of single group(t=5.692,7.835,9.195,6.391,8.316;P<0.05).5 days after treatment,the levels of IgM,IgG and IgA in combined group were higher than those in single group(t=4.284,12.138,2.124;P<0.05).TNF-a,IL-6,IL-1βand IL-10 were significantly lower than those in single group(t=19.491,7.799,27.673,11.194;P<0.05).Conclusion:Vidarabine monophosphatate combined with interferon in children with severe hand,foot and mouth disease can shorten the time of symptom improvement,improve the level of immunoglobulin,reduce the level of inflammatory factors,and reduce the incidence of adverse reactions.
