归脾汤加味联合黛力新治疗心脾两虚型缺血性脑卒中后抑郁临床研究

摘　　要：目的:观察归脾汤加味联合黛力新治疗心脾两虚型缺血性脑卒中后抑郁患者的疗效。方法:选取心脾两虚型缺血性脑卒中后抑郁患者80例,随机分为2组,每组40例。对照组予口服黛力新片治疗,中药组在对照组基础上加服归脾汤加味,两组疗程均为4周,观察比较两组治疗前后美国国立卫生院卒中量表(NIHSS)评分、汉密尔顿抑郁量表(HAMD)评分、健康调查简表(SF-36)评分、血清5-羟色胺(5-HT)水平、血清同型半胱氨酸(Hcy)水平变化情况及临床疗效、不良反应发生率。结果:治疗后,两组患者HAMD评分、SF-36评分、NIHSS评分及5-HT、Hcy水平均较治疗前改善(P<0.05),且中药组较对照组改善明显,差异具有统计学意义(P<0.05)。中药组总有效率为95.00%,高于对照组的80.00%,差异具有统计学意义(P<0.05)。中药组不良反应总发生率为5.0%,低于对照组的17.5%(P<0.05)。结论:归脾汤加味联合黛力新可改善心脾两虚型缺血性脑卒中后抑郁患者抑郁状态,安全性较好。Objective:To observe the therapeutic effect of the supplemented Guipi decoction combined with Deanxit in the treatment of ischemic post-stroke depression(IPSD)of deficiency of the heart and spleen type.Methods:A total of 80 patients with IPSD of deficiency of the heart and spleen type were randomly divided into two groups,with 40 cases in each group.The observation group received treatment with Deanxit,while the traditional Chinese medicine(TCM)group was treated with supplemented Guipi decoction on the basis of the control group.The course of treatment for both groups was four weeks.Evaluation of NIHSS scores,HAMD scores,SF-36 scores,5-hydroxytryptamine(5-HT)levels,homocysteine(Hcy)levels,clinical effects,and the incidence of adverse reactions was conducted in two groups before and after treatment.Results:After treatment,the HAMD scores,SF-36 scores,NIHSS scores,and levels of 5-HT and Hcy all showed improvement in both groups(P<0.05).Furthermore,the TCM group demonstrated a more significant improvement compared to the control group,with statistically significant differences(P<0.05).The total effective rate in the TCM group was 95.00%,surpassing the 80.00%observed in the control group,and the difference was statistically significant(P<0.05).The incidence of adverse reactions in the TCM group was 5.0%,lower than the 17.5%observed in the control group(P<0.05).Conclusion:The combination of supplemented Guipi decoction and Deanxit can improve the depressive symptoms in patients with IPSD of deficiency of the heart and spleen type,with a high level of safety.
