云南白药胶囊联合肾上腺色腙治疗肺癌咯血的疗效观察

摘　　要：目的分析云南白药胶囊联合肾上腺色腙治疗肺癌咯血的临床疗效。方法选取太和县人民医院2021年10月—2023年9月住院的88例肺癌伴咯血患者,利用随机数字表法进行分组,对照组和治疗组各44例。对照组:肾上腺色腙片,口服,5 mg/次,3次/d。治疗组在对照组治疗基础上加用云南白药胶囊,口服,0.5 g/次,3次/d。两组疗程均为7 d。比较两组临床疗效、咯血总量和止血时间及治疗前后简式抑郁–焦虑–压力量表(DASS-21)、36项健康调查简表(SF-36)评分变化。结果治疗后,治疗组总有效率是93.18%,显著高于对照组的77.27%(P<0.05)。治疗后,治疗组咯血总量显著少于对照组,止血时间较对照组明显缩短(P<0.05)。治疗后,两组患者DASS-21中抑郁、焦虑、压力评分及总评分均显著降低(P<0.05);且DASS-21中各维度评分及总评分均以治疗组显著更低(P<0.05)。两组SF-36评分(包括总分及生理、心理健康总分)均于治疗后显著升高(P<0.05),且SF-36中各领域总分及量表总评分均以治疗组显著更高(P<0.05)。结论云南白药胶囊联合肾上腺色腙治疗肺癌咯血的总体效果确切,能快速止血,有效减少咯血量,安全性较好,且利于改善患者心理状态及生活质量。Objective To analyze the clinical efficacy of Yunnan Baiyao Capsules combined with carbazochrome in treatment of hemoptysis in lung cancer.Methods A total of 88 patients with lung cancer hemoptysis admitted to Taihe County People's Hospital from October 2021 to September 2023 were selected and divided into control group and treatment group according to random number table method,with 44 cases in each group.Patients in the control group were po administered with Carbazochrome Tablets,5 mg/time,three times daily.Patients in the treatment group were po administered with Yunnan Baiyao Capsules on the basis of the control group,0.5 g/time,three times daily.The treatment course of both groups was 7 d.The clinical effect,total amount of hemoptysis and hemostasis time of the two groups were compared,and the changes of the DASS-21 and 36-item health survey summary Form(SF36)scores before and after treatment were compared.Results After treatment,the total effective rate of treatment group was 93.18%,which was significantly higher than that of control group(77.27%)(P<0.05).After treatment,the total amount of hemoptysis in treatment group was significantly less than that in control group,and the hemostasis time was significantly shorter than that in control group(P<0.05).After treatment,the scores of depression,anxiety,stress and the total score of DASS-21 in both groups were significantly decreased(P<0.05).The score of all dimensions and the total score of DASS-21 were significantly lower in treatment group(P<0.05).The SF-36 scores(including total score and overall physiological and mental health scores)of both groups were significantly increased after treatment(P<0.05),and the SF-36 scores in all areas and the total score of the scale were significantly higher than those of treatment group(P<0.05).Conclusion Yunnan Baiyao Capsules combined with carbazochrome has exact overall effect in treatment of hemoptysis in lung cancer,and can quickly stop bleeding,effectively reduce the amount of hemoptysis with good safety,which is
