摘 要:目的报道应用国产单孔蛇形臂腹腔镜机器人(术锐SR-ENS-600)进行机器人单孔腹腔镜手术治疗妇科良性疾病的初步经验,并探讨该手术方式的安全性和可行性。方法回顾性分析2022年4月至2023年4月北京协和医院普通妇科中心21例应用术锐SR-ENS-600系统手术的患者临床资料资料和42例应用单孔腹腔镜进行手术的患者临床资料,通过评估手术时间、术中出血量、住院时间、术中及术后并发症、伤口愈合情况等多项因素对围手术期结果进行比较。结果R-LESS(robotic-assited laparoscopic single-site surgery,R-LESS)组和LESS(laparoendoscopic single-site surgery,LESS)组的平均手术时间分别为99.00(53.00,130.00)min和50.00(40.00,70.00)min,平均术中失血量为30.00(20.00,80.00)ml和50.00(50.00,100.00)ml。所有手术均通过单孔手术方式成功完成,未改变手术方式。R-LESS组在手术时间、住院时间、血红蛋白变化等指标方面均优于LESS组。结论应用术锐SR-ENS-600单孔腹腔镜机器人系统治疗妇科良性疾病是安全可行的。Objective To report the preliminary experience of robotic single-port laparoscopic surgery for benign gynecologic surgery by applying a domestic single-port serpentine-arm laparoscopic robot(SR-ENS-600)and to discuss the safety and feasibility of this surgical approach.Methods The clinical data of 21 patients who underwent the SR-ENS-600 single-port robotic surgery and 42 patients who were underwent the single-port laparoscopic surgery at the General Gynecology Center of Peking Union Medical College Hospital during the period of Apr.2022 to Apr.2023 were retrospectively analyzed,and the perioperative outcomes,inclueded operative time,intraoperative bleeding,hospital stay,intra-operative and postoperative complications,wound healing,were compared.Results The mean operative times in the R-LESS and LESS groups were 99.00(53.00,130.00)and 50.00(40.00,70.00)minutes,respectively,and the mean intraoperative blood loss was 30.00(20.00,80.00)ml and 50.00(50.00,100.00)ml.All surgeries were successfully completed,without changing the surgical approach.The R-LESS group was higher than the LESS group in terms of surgical time,hospitalization time,and hematocrit changes.Conclusion Robotic single-port surgery using the SR-ENS-600 surgical system might be a safe and feasible surgical technique for various gynecological benign diseases.
