行造血干细胞采集的健康供者外周血CD34^(+)细胞采集数量相关因素分析

摘　　要：目的探讨行造血干细胞采集的健康供者外周CD34^(+)细胞采集数量的相关因素。方法回顾性病例系列研究。回顾性分析2020年6月至2021年6月苏州大学附属儿童医院行异基因外周血造血干细胞采集的94名健康供者信息。健康供者注射重组人粒细胞集落刺激因子,动员后开始采集。统计健康供者动员前血常规和动员前CD34^(+)(preCD34^(+))细胞比例,及外周血干细胞采集物中白细胞计数(WBC)、CD34^(+)、总CD34^(+)细胞数。采用Spearman相关分析法分析采集物中总CD34^(+)细胞与健康供者动员前信息的相关性。结果94名健康供者动员前外周血WBC中位数[M(Q1,Q3)]为6.13×10^(9)/L(5.29×10^(9)/L,7.01×10^(9)/L);红细胞比容(HCT)中位数为44.4%(40.3%,47.6%);血小板计数(Plt)中位数为250.5×10^(9)/L(207.0×10^(9)/L,297.8×10^(9)/L);preCD34^(+)细胞比例中位数为0.75%(0.57%,1.13%)。外周血干细胞采集物中,总WBC中位数为250.86×10^(9)/L(195.21×10^(9)/L,315.60×10^(9)/L);CD34^(+)细胞中位数为8.38×10^(6)/kg(4.25×10^(6)/kg,13.27×10^(6)/kg);总CD34^(+)细胞数中位数为217.58×10^(6)个(135.68×10^(6)个,330.41×10^(6)个)。采集物中总CD34^(+)细胞数与健康供者年龄呈正相关(r=0.22,P<0.05);与动员前HCT呈正相关(r=0.25,P<0.05);与动员前Plt呈正相关(r=0.21,P<0.05);与preCD34^(+)细胞比例无相关性(r=0.01,P>0.05);与动员前WBC无相关性(r=0.08,P>0.05)。结论健康供者年龄、动员前HCT和Plt与采集物总CD34^(+)细胞数呈正相关;preCD34^(+)细胞比例、动员前WBC与采集物总CD34^(+)细胞数无相关性。Objective To explore the factors associated with the number of peripheral CD34^(+)cells collected from healthy donors for hematopoietic stem cell collection.Methods A retrospective case series study was conducted.The data of 94 healthy donors who underwent allogeneic peripheral blood hematopoietic stem cell collection from June 2020 to June 2021 at Children's Hospital of Soochow University was retrospectively analyzed.Healthy donors were injected with recombinant human granulocyte colony-stimulating factor,and collection began after mobilization.The pre-mobilization blood routine and the proportion of pre-mobilization CD34^(+)(preCD34^(+))cells,as well as the white blood cell count(WBC)and the number of CD34^(+)and total CD34^(+)cells in peripheral blood stem cell collections were counted in the healthy donors.Spearman correlation analysis was used to analyze the correlation between total CD34^(+)cells in the collections and pre-mobilization information of healthy donors.Results The median[M(Q1,Q3)]pre-mobilization peripheral blood WBC of 94 healthy donors was 6.13×10^(9)/L(5.29×10^(9)/L,7.01×10^(9)/L);the median hematocrit(HCT)was 44.4%(40.3%,47.6%);the median platelet count(Plt)was 250.5×10^(9)/L(207.0×10^(9)/L,297.8×10^(9)/L);the median percentage of preCD34^(+)cells was 0.75%(0.57%,1.13%).In peripheral blood stem cell collections,the median total WBC was 250.86×10^(9)/L(195.21×10^(9)/L,315.60×10^(9)/L);the median CD34^(+)cell count was 8.38×10^(6)/kg(4.25×10^(6)/kg,13.27×10^(6)/kg);the median total CD34^(+)cell count was 217.58×10^(6)(135.68×10^(6),330.41×10^(6)).The total CD34^(+)cell count in the collections was positively correlated with the age of healthy donors(r=0.22,P<0.05),the pre-mobilization HCT(r=0.25,P<0.05)and the pre-mobilization Plt(r=0.21,P<0.05);it did not correlate with the proportion of preCD34^(+)cells(r=0.01,P>0.05)or the pre-mobilization WBC(r=0.08,P>0.05).Conclusions Healthy donor's age,pre-mobilization HCT and Plt are positively correlated with total CD34^(+)cell count i
