血栓心脉宁联合单硝酸异山梨酯注射液治疗冠心病不稳定型心绞痛的效果观察

摘　　要：目的:探讨血栓心脉宁联合单硝酸异山梨酯注射液治疗冠心病不稳定型心绞痛(UAP)的疗效及对心肌肌钙蛋白I(c TnI)、N末端B型利钠肽前体(NT-proBNP)的影响。方法:选择2020年6月—2022年6月西平县中医院心内科收治的100例冠心病UAP患者,随机分为参照组和观察组,各50例。参照组予以单硝酸异山梨酯注射液治疗,观察组给予单硝酸异山梨酯注射液+血栓心脉宁治疗。比较两组治疗前后血清cTnI、NT-proBNP水平、6 min步行实验、临床疗效及心电图疗效。结果:观察组临床总有效率高于参照组(P<0.05);观察组心电图总有效率高于参照组,但差异无统计学意义(P>0.05);观察组治疗后NT-proBNP水平低于参照组(P<0.05),但两组cTnI比较,差异无统计学意义(P>0.05);观察组6 min步行距离长于对照组(P<0.05)。结论:与单硝酸异山梨酯单药治疗相比,联合血栓心脉宁治疗冠心病不稳定型心绞痛具有一定的协同增效作用,可有效改善部分心脏生物标志物,更好地控制心绞痛症状,提高活动耐力,但对心电图改善效果有限。Objective:To investigate the efficacy of Xueshuan Xinmaining combined with isosorbide mononitrate injection in the treatment of unstable angina pectoris(UAP)of coronary heart disease and its effect on cardiac troponin I(cTnI)and N-terminal B-type natriuretic peptide precursor(NT-proBNP).Methods:A total of 100 patients with UAP of coronary heart disease admitted to Department of Cardiology,Xiping County Hospital of Traditional Chinese Medicine from June 2020 to June 2022 were randomly divided into control group and observation group,with 50 patients in each group.The reference group was treated with isosorbide mononitrate injection,and the observation group was treated with isosorbide mononitrate injection+Xueshuan Xinmaining.The levels of serum cTnI and NT-proBNP,6 min walking test,clinical efficacy and electrocardiogram efficacy were compared between the two groups before and after treatment.Results:The total clinical effective rate of the observation group was higher than that of the reference group(P<0.05).The total effective rate of electrocardiogram in the observation group was higher than that in the reference group,but the difference was not statistically significant(P>0.05).After treatment,the level of NT-proBNP in the observation group was lower than that in the reference group(P<0.05).However,there was no significant difference in cTnI between the two groups(P>0.05).The 6 min walking distance in the observation group was longer than that in the control group(P<0.05).Conclusion:Compared with isosorbide mononitrate monotherapy,combined with Xueshuan Xinmaining in the treatment of unstable angina pectoris of coronary heart disease has a certain synergistic effect,which can effectively improve some cardiac biomarkers,better control angina symptoms,and improve activity endurance,but the effect on electrocardiogram improvement is limited.
