足底静脉泵对肿瘤化疗患者住院期间下肢深静脉血栓的预防价值

摘　　要：目的探讨足底静脉泵对肿瘤化疗患者住院期间下肢深静脉血栓(DVT)的预防价值。方法收集2021年2月至2023年1月于亳州市人民医院住院接受化疗的128例恶性肿瘤患者的临床资料,根据住院期间是否接受了足底静脉泵干预将患者分为干预组(n=67)和对照组(n=61)。比较两组患者住院期间主要治疗信息、住院期间结局指标。结果两组患者各种治疗方案的使用比例比较,差异均无统计学意义(P﹥0.05)。住院期间,干预组患者下肢DVT的发生率和D-二聚体水平均低于对照组患者,差异均有统计学意义(P﹤0.05)。两组患者的凝血酶原时间、活化部分凝血活酶时间、消化道症状及白细胞计数减少的发生率比较,差异均无统计学意义(P﹥0.05)。干预组患者未发生足底静脉泵治疗相关并发症。结论足底静脉泵可有效降低肿瘤化疗患者住院期间发生下肢DVT的风险,且不会出现明显的治疗相关并发症,对患者的凝血功能无明显影响,值得临床推广。Objective To investigate the value of plantar venous pump in the prevention of lower extremity deep vein thrombosis(DVT)in patients with tumor chemotherapy during hospitalization.Method The clinical data of 128 malignant tumor patients who received chemotherapy in the People's Hospital of Bozhou from February 2021 to January 2023 were collected.According to whether they received plantar venous pump intervention during hospitalization,the patients were divided into intervention group(n=67)and control group(n=61).The main treatment information and outcome indicators during hospitalization between two groups of patients were compared.Result There were no statistically significant difference in the proportion of different treatment plans used between the two groups of patients(P>0.05).During hospitalization,the incidence of lower extremity DVT and D-dimer levels in the intervention group were lower than those in the control group,and the differences were statistically significant(P<0.05).There were no statistically significant differences in the incidences of prothrombin time,activated partial thromboplastin time,gastrointestinal symptoms,and the incidence of decreased white blood cell count between the two groups of patients(P>0.05).The intervention group did not experience any complications related to plantar venous pump treatment.Conclusion The plantar vein pump can effectively reduce the risk of lower extremity DVT in tumor chemotherapy patients during hospitalization,and no obvious treatment related complications were found.It has no significant impact on the patient's coagulation function and is worthy of clinical promotion.
