低分子肝素联合地屈孕酮治疗先兆流产合并宫腔积血的临床疗效研究

摘　　要：探究对先兆流产合并宫腔积血患者实施低分子肝素联合地屈孕酮治疗的效果.选取2021年1月-2022年12月甘肃医学院附属医院收治的100例先兆流产合并宫腔积血患者为研究对象,随机将其分为对照组(n=50)和研究组(n=50),对照组患者接受地屈孕酮治疗,研究组患者在对照组治疗基础上接受低分子肝素治疗,对比两组患者治疗效果.结果显示,治疗后,研究组血肿面积小于对照组、孕酮、HCG以及E2水平均高于对照组(P<0.05);治疗后,研究组患者的凝血-纤溶指标水平和炎症因子水平优于对照组(P<0.05);治疗后,研究组患者PSV、EDV参数均高于对照组,动脉RI参数低于对照组(P<0.05);研究组妊娠成功率高于对照组(P<0.05);两组患者不良反应总发生率比较无统计学差异(P>0.05).研究发现,对先兆流产合并宫腔积血患者采取低分子肝素联合地屈孕酮治疗,可提高患者的妊娠成功率,值得推广.Exploring the effect of low-molecular-weight heparin combined with dexamethasone on patients with threatened miscarriage and uterine bleeding.100 patients with threatened miscarriage and uterine bleeding admitted to the Affiliated Hospital of Gansu Medical University from January 2021 to December 2022 were selected as the study subjects.They were randomly divided into a control group(n=50)and a study group(n=50).The control group received treatment with dexmedetone,while the study group received treatment with low molecular weight heparin on the basis of the control group.The treatment effects of the two groups were compared.The results showed that after treatment,the hematoma area of the study group was smaller than that of the control group,and the levels of progesterone,HCG,and E2 were higher than those of the control group(P<0.05);After treatment,the levels of coagulation fibrinolysis indicators and infl ammatory factors in the study group were better than those in the control group(P<0.05);After treatment,the PSV and EDV parameters of the study group patients were higher than those of the control group,while the arterial RI parameters were lower than those of the control group(P<0.05);The success rate of pregnancy in the study group was higher than that in the control group(P<0.05);There was no statistically signifi cant difference in the total incidence of adverse reactions between the two groups of patients(P>0.05).Research has found that the combination of low-molecular-weight heparin and dexamethasone can improve the success rate of pregnancy in patients with threatened miscarriage and uterine bleeding,and is worth promoting.
