腹腔镜下胃十二指肠溃疡穿孔修补术40例临床分析

摘　　要：目的 探讨腹腔镜下胃十二指肠溃疡穿孔修补术与开腹手术的临床疗效。方法 选取80例胃十二指肠溃疡穿孔患者,根据手术方式将其分为观察组(腹腔镜手术)和对照组(开腹手术),每组40例,比较两组患者术后的临床指标、胃肠道情况、血清因子水平及不良反应。结果 观察组患者术后1 h SP、PGE2、TFN-α、IL-10、IL-6水平优于对照组(P<0.05);手术指标及并发症发生率明显优于对照组(P<0.05)。结论 腹腔镜下胃十二指肠溃疡穿孔修补术相对比较安全,该术式能够缩短患者恢复时间,降低术后并发症的发生率,值得临床推广应用。Objective To explore the clinical efficacy of laparoscopic gastroduodenal ulcer perforation repair and laparotomy.Method 80 patients with gastroduodenal ulcer perforation were divided into observation group(laparoscopic)and control group(open abdomen)according to the operation method,with 40 patients in each group.The clinical index and gastrointestinal recovery index,serum pain-related substances and the occurrence of adverse reactions in two groups were compared.Results The surgical indicators and complication rates of the observation group were significantly superior to those of the control group(P<0.05);The levels of SP,PGE2,TFN-α,IL-10 and IL-6 at 1 h after operation in the observation group were better than those in the control group(P<0.05).Conclusion Laparoscopic gastroduodenal ulcer perforation repair is relatively safe,which can shorten the recovery time of patients and reduce postoperative complications,which is worthy of clinical application.
