摘 要:目的探讨3D设计与打印技术在儿童发育性髋脱位(DDH)矫形术中的应用效果。方法选取2020年1月至2021年9月宜春市人民医院收治的20例DDH患儿作为研究对象,根据治疗方式的不同分为研究组与对照组,每组10例。对照组采用常规手术治疗,研究组采用3D设计与打印技术进行手术治疗,比较两组围手术期指标、形态学参数、治疗前后Harris髋关节功能评分(Harris评分)及肢体运动功能[Fugl-Meyer肢体运动功能评分(FMA)]、治疗优良率及并发症发生情况。结果研究组手术时间短于对照组,术中出血量、辐射次数均少于对照组,差异有统计学意义(P<0.05)。两组股骨前倾角、髋臼短轴、髋臼长轴、髋臼表面积、髋臼容积比较差异均无统计学意义。治疗后,两组Harris、FMA评分均高于治疗前,且研究组高于对照组,差异有统计学意义(P<0.05)。两组Mackay临床评价分级、Severin髋关节X线分级优良率比较差异无统计学意义。两组并发症发生率比较差异无统计学意义。结论DDH手术治疗中应用3D设计与打印技术效果显著,可有效促进患儿术后髋关节功能与运动功能恢复。Objective To investigate the application of 3D design and printing technology in the orthopedic of developmental dislocation of the hip(DDH)in children.Methods 20 patients with DDH treated in Yichun People's Hospital from January 2020 to September 2021 were selected as the research subjects,and they were divided into the study group and the control group according to different treatment methods,with 10 cases in each group.The control group was treated with conventional surgery,and the study group was treated with 3D design and printing technology,the perioperative indexes,morphological parameters,Harris hip function score(Harris score)and limb motor function(Fugl-Meyer motor function score[FMA])before and after treatment,the rate of excellent and good treatment and the incidence of complications were compared between the two groups.Results The operation time in the study group was shorter than that in the control group,the amount of blood loss and the number of radiation were less than those in the control group,the differences were statistically significant(P<0.05).There were no significant differences in femoral anterior inclination,acetabular minor axis,acetabular major axis,acetabular surface area and acetabular volume between the two groups.Af-ter treatment,the Harris and FMA scores of the two groups were higher than those before treatment,and the study group was higher than the control group,the differences were statistically significant(P<0.05).There was no significant difference in the excellent and good rates of Mackay clinical eraluation grade and Severin hip X-ray grade between the two groups.There was no statistically significant difference in the total incidence of com-plications between the two groups.Conclusion The application of 3D design and printing technology in DDH surgery has significant effects,which can effectively promote the recovery of hip function and motor function of children after surgery.
