摘 要:目的分析高频超声在手部不同软组织包块诊断中的价值。方法选取2020年1月至2022年1月瑞金市人民医院接诊的72例手部软组织包块患者作为研究对象。所有患者均采用高频超声检查,以病理诊断结果为金标准,比较不同包块类型患者高频超声检查诊断结果,观察高频超声检查的诊断效能。结果病理检查诊断结果显示,72例患者中,腱鞘巨细胞瘤9例,血管瘤7例,腱鞘纤维瘤18例,表皮样囊肿13例,腱鞘囊肿14例,脂肪瘤11例。将腱鞘巨细胞瘤、血管瘤、表皮样囊肿、腱鞘囊肿、脂肪瘤归为常见软组织包块,患者54例;将腱鞘纤维瘤作为罕见软组织包块,患者18例。高频超声检查诊断常见软组织包块诊断符合率高于罕见软组织包块,差异有统计学意义(P<0.05)。以病理检查诊断结果为金标准,72例患者中,良性肿块患者67例,恶性肿块5例。高频超声检查诊断手部软组织包块良恶性的灵敏度为97.01%,特异度为80.00%,准确度为95.83%,阳性预测值为98.48%,阴性预测值为66.67%。结论高频超声检测技术在手部不同软组织包块患者的诊断中能有效确诊各类常见软组织包块问题,但对于罕见的软组织包块,仅能为患者病情的确诊提供部分指导意见,对肿块良恶性诊断效能较好,对手部软组织包块患者的诊断具有重要意义。Objective Analysis of the value of high-frequency ultrasound in the diagnosis of different soft tissue masses of the hand.Methods 72 patients with hand soft tissue mass admitted to Ruijin People's Hospital from January 2020 to January 2022 were selected as the study subjects.All patients were examined by high-frequency ultrasound,the results of pathological diagnosis were used as the gold standard,the results of high-fre-quency ultrasound examination in patients with different mass types were compared,and the diagnostic efficacy of high-frequency ultrasound exami-nation was observed.Results Pathological examination showed that among the 72 patients,there were 9 cases of giant cell tumor of tendon sheath,7 cases of hemangioma,18 cases of fibrous tumor of tendon sheath,13 cases of epidermoid cyst,14 cases of tendon sheath cyst and 11 cases of lipo-ma.Giant cell tumor of tendon sheath,hemangioma,epidermoid cyst,ganglion cyst and lipoma were classified as common soft tissue masses in 54 patients;tendon sheath fibroma was treated as a rare soft tissue mass in 18 patients.The diagnostic coincidence rate of high-frequency ultrasound in the diagnosis of common soft tissue masses was higher than that of rare soft tissue masses,and the difference was statistically significant(P<0.05).With the pathological diagnosis as the gold standard,there were 67 cases of benign masses and 5 cases of malignant masses in 72 patients.The sensi-tivity,specificity,accuracy,positive predictive value and negative predictive value of high frequency ultrasonography in the diagnosis of benign and malignant hand soft tissue masses were 97.01%,80.00%,95.83%,98.48%and 66.67%,respectively.Conclusion High-frequency ultrasound detec-tion technology can effectively diagnose various common soft tissue masses in the diagnosis of patients with different soft tissue masses in the hand.However,for rare soft tissue masses,it can only provide some guidance for the diagnosis of the patient's condition,the diagnosis of benign and malignant masses in patients wi
