摘 要:急性肾损伤(AKI)是围术期常见的并发症,常见于创伤后或感染后的患者,其主要特点是肾小球滤过率急剧下降,肾小管功能受损,导致体内代谢产物的积累和电解质紊乱。AKI发病机制与炎症反应相关, Toll样受体4(TLR4)在炎症反应中起到病原体识别和先天免疫激活的作用,其介导的炎症反应在急性肾损伤的发病机制中起着关键作用。临床上可通过干预TLR4介导的炎症反应进行早期治疗,从而有效降低急性肾损伤的发生率,延缓疾病进展,改善患者预后。本文将从TLR4结构和配体、TLR4表达、TLR4的细胞内信号转导、靶向TLR4的分子化合物等研究进展进行综述。Acute kidney injury(AKI)is a common perioperative complication,which is common in patients after trauma or infection.Its main feature is a sharp decline in glomerular filtration rate,impaired renal tubular function,resulting in the accumulation of metabolites and electrolyte disorders in the body.The pathogenesis of AKI is related to the inflammatory response.Toll-like receptor 4(TLR4)plays a role in pathogen recognition and innate immune activation in the inflammatory response,and its mediated inflammatory response plays a key role in the pathogenesis of acute kidney injury.Clinically,early treatment can be performed by interfering with the TLR4-mediated inflammatory response,thereby effectively reducing the incidence of acute kidney injury,delaying disease progression,and improving the prognosis of patients.This article will review the research progress of TLR4 structure and ligand,TLR4 expression,TLR4 intracellular signal transduction,and molecular compounds targeting TLR4.
