腰髂部触发点内热针与冲击波治疗腰源性腹痛的效果

摘　　要：目的观察腰髂部触发点内热针与冲击波治疗腰源性腹痛的效果。方法选取50例腰源性腹痛患者,随机将其分为观察组和对照组,各25例。观察组采用腰髂部触发点内热针治疗,对照组采用腰髂部触发点的冲击波治疗。比较两组的治疗效果。结果疗程结束时及治疗后3、6个月,观察组的视觉模拟评分法(VAS)评分低于治疗前(P<0.05);疗程结束时,对照组的VAS评分低于治疗前(P<0.05);治疗后3、6个月,观察组的VAS评分低于对照组(P<0.05)。治疗后3个月,两组的简明健康状况调查表(SF-36)各维度评分均显著高于治疗前(P<0.05);治疗后3个月,观察组的躯体疼痛、精力及社会功能评分明显高于对照组(P<0.05)。观察组的治疗总有效率显著高于对照组(P<0.05)。两组患者治疗过程中均未发生不良反应,未服用镇痛药物。结论腰髂部触发点内热针与冲击波疗法短期内均能有效缓解疼痛,但前者较后者治疗腰源性腹痛的远期效果更好。Objective To observe the effects of internal heat acupuncture and shock wave at lumbar iliac trigger point in the treatment of lumbar abdominal pain.Methods Fifty patients with lumbar abdominal pain were selected and randomly divided into observation group and control group,with 25 cases in each group.The observation group was treated with internal heat acupuncture at lumbar iliac trigger point,and the control group was treated with shock wave at lumbar iliac trigger point.The therapeutic effects of the two groups were compared.Results At the end of treatment and 3 and 6 months after treatment,the Visual Analog Scale(VAS)score of the observation group was lower than that before treatment(P<0.05);at the end of treatment,the VAS score of the control group was lower than that before treatment(P<0.05);at 3 and 6 months after treatment,the VAS score of the observation group was lower than that of the control group(P<0.05).At 3 months after treatment,the scores of each dimension of Medical Outcomes Study 36-Item Short-Form Health Survey(SF-36)in the two groups were significantly higher than those before treatment(P<0.05);at 3 months after treatment,the scores of physical pain,vitality and social function in the observation group were significantly higher than those in the control group(P<0.05).The total effective rate of treatment in the observation group was significantly higher than that in the control group(P<0.05).No adverse reactions occurred in the two groups of patients during the treatment,and no analgesic drugs were taken.Conclusion Both internal heat acupuncture and shock wave at lumbar iliac trigger point can effectively relieve pain in the short term,but the former has better long-term efficacy than the latter in the treatment of lumbar abdominal pain.
