基于中医升降理论的辨证施治在肝胃气滞型慢性萎缩性胃炎中的应用效果

摘　　要：目的分析基于中医升降理论的辨证施治在肝胃气滞型慢性萎缩性胃炎中的应用效果。方法选取2021年10月至2022年10月收治的100例肝胃气滞型慢性萎缩性胃炎患者为研究对象,以随机数字表法将其分为对照组和观察组,各50例。对照组给予西医常规治疗,观察组在对照组基础上行基于中医升降理论的辨证施治。比较两组的治疗效果。结果观察组的治疗总有效率高于对照组(P<0.05)。治疗后,两组的中医证候积分及改良Mayo活动指数均降低,且观察组低于对照组(P<0.05)。治疗后,两组的胃蛋白酶原Ⅰ(PGⅠ)、胃蛋白酶原Ⅱ(PGⅡ)水平及胃蛋白酶原比值(PGR)均升高,且观察组高于对照组(P<0.05)。观察组的幽门螺杆菌(Hp)根除率高于对照组(P<0.05)。治疗后,两组的全身症状、肠道症状、社会功能、精神健康评分及总分均升高,且观察组高于对照组(P<0.05)。结论基于中医升降理论的辨证施治在肝胃气滞型慢性萎缩性胃炎中的应用效果显著,可改善患者的临床症状,提高Hp根除率,促进胃肠功能恢复,最终提升生活质量,值得推广。Objective To analyze the application effect of syndrome differentiation and treatment based on the traditional Chinese medicine ascending and descending theory in chronic atrophic gastritis of liver and stomach Qi stagnation type.Methods From October 2021 to October 2022,100 patients with chronic atrophic gastritis of liver and stomach Qi stagnation type were selected as the research objects.The patients were divided into control group and observation group by random number table method,with 50 cases in each group.The control group was treated with conventional western medicine,and the observation group received syndrome differentiation and treatment based on the traditional Chinese medicine ascending and descending theory on the basis of the control group.The therapeutic effects of the two groups were compared.Results The total effective rate of treatment in the observation group was higher than that in the control group(P<0.05).After treatment,the traditional Chinese medicine syndrome score and modified Mayo activity index of the two groups decreased,and those in the observation group were lower than the control group(P<0.05).After treatment,the levels of pepsinogenⅠ(PGⅠ),pepsinogenⅡ(PGⅡ)and pepsinogen ratio(PGR)in the two groups increased,and those in the observation group were higher than the control group(P<0.05).The eradication rate of Helicobacter pylori(Hp)in the observation group was higher than that in the control group(P<0.05).After treatment,the scores of systemic symptom,intestinal symptom,social function,mental health and total score of the two groups increased,and those in the observation group were higher than the control group(P<0.05).Conclusion The syndrome differentiation and treatment based on the traditional Chinese medicine ascending and descending theory applied in chronic atrophic gastritis of liver and stomach Qi stagnation type has significant effect,it can improve the clinical symptoms of patients,increase the eradication rate of Hp,promote the recovery of gastrointestinal funct
