摘 要:目的探讨血清细胞外基质蛋白-3(Fibulin-3)、生长停滞特异基因6(Gas6)、对氧磷酶-1(PON-1)联合检测对重度子痫前期(PE)患者发生胎盘早剥的预测价值。方法回顾性抽取2020年3月至2023年8月郑州颐和医院收治的发生胎盘早剥的23例PE患者为早剥组,另抽取同期未发生胎盘早剥的46例PE患者为非早剥组。比较两组入院24 h血清Fibulin-3、Gas6、PON-1水平,分析三项指标联合检测预测胎盘早剥的效能,分析入院24 h三项指标不同水平者发生胎盘早剥的危险度。结果早剥组入院24 h血清Fibulin-3、Gas6、PON-1水平低于非早剥组(P<0.05)。血清Fibulin-3、Gas6、PON-1联合检测对胎盘早剥的预测效能曲线下面积为0.914。入院24 h血清Fibulin-3、Gas6、PON-1高水平者发生胎盘早剥的危险度为低水平者的0.127、0.272、0.190倍(P<0.05)。结论血清Fibulin-3、Gas6、PON-1水平与PE患者发生胎盘早剥密切相关,三项联合检测的预测效能良好,可为临床早期预测、制定预防方案提供参考。Objective To investigate the predictive value of combined detection of serum extracellular matrix protein-3(fibulin-3),growth arrest specific gene 6(Gas6)and paraoxophosphorase-1(PON-1)for placental abruption in patients with severe preeclampsia(PE).Methods A total of 23 PE patients with placental abruption admitted to Zhengzhou Yihe Hospital from March 2020 to August 2023 were retrospectively selected as the abruption group,and 46 PE patients who did not have placental abruption during the same period were selected as the non-abruption group.The serum levels of fibulin-3,Gas6 and PON-1 within 24 hours after admission were compared between the two groups.The predictive performance for placental abruption of the combined detection of the three indicators was analyzed.The risk of placental abruption in patients with different levels of the three indicators within 24 hours after admission was analyzed.Results The serum levels of fibulin-3,Gas6 and PON-1 within 24 hours after admission in the abruption group were lower than those in the non-abruption group(P<0.05).The area under the curve of predictive efficacy for placental abruption of the combined detection of serum fibulin-3,Gas6 and PON-1 was 0.914.The risk of placental abruption in patients with high serum levels of fibulin-3,Gas6 and PON-1 within 24 hours after admission were 0.127,0.272 and 0.190 times higher than those of patients with low levels(P<0.05).Conclusions The serum levels of fibulin-3,Gas6 and PON-1 are closely related to placental abruption in PE patients.The prediction performance of the combination of the three indicators is good,which can provide reference for early clinical prediction and formulation of prevention programs.
