摘　　要：目的探讨无创血流动力学检测指标对心力衰竭易损期预后的预测价值。方法随机选取2021年11月至2022年11月于承德市中心医院住院的慢性心力衰竭患者85例,记录患者出院后3个月药物治疗、再就诊、再住院和死亡等情况。采用多因素logistic回归分析,分析影响心力衰竭易损期预后的因素。结果共完成随访85例患者,预后不良26例。预后不良组患者的左心室射血分数(LVEF)、心输出量(CO)、心脏指数(CI)、左心室做功(LCW)均低于预后良好组,差异有统计学意义(P<0.05);预后不良组患者的胸液水平(TFC)、外周阻力(SVR)高于预后良好组,差异有统计学意义(P<0.05)。其中LVEF、LCW、TFC是慢性心力衰竭易损期的预后预测因素(P<0.05)。结论无创血流动力学检测指标中LVEF、LCW、TFC是心力衰竭患者易损期重要的预测因素,对心力衰竭易损期的预后或可具有预测价值,对慢性心力衰竭患者临床治疗具有指导意义。Objective To investigate the predictive value of noninvasive hemodynamics testing indices on prognosis of vulnerable stage of heart failure.Methods A total of 85 patients with chronic heart failure hospitalized in the Chengde Central Hospital from November 2021 to November 2022 were randomly selected,and their medication treatment,re-visit,re-hospitalization and death within 3 months after discharge were recorded.Multivariate logistic regression analysis was used to analyze the factors impacting the prognosis of vulnerable stage of heart failure.Results A total of 85 patients were followed up,in which 26 patients had poor prognosis.The left ventricular ejection fraction(LVEF),cardiac output(CO),cardiac index(CI)and left ventricular stroke work(LCW)in patients with poor prognosis were all lower than those in patients with good prognosis,with statistically significant differences(P<0.05).The level of thoracic fluid content(TFC)and systemic vascular resistance(SVR)in patients with poor prognosis were higher than those in patients with good prognosis,with statistically significant differences(P<0.05).In the above,LVEF,LCW and TFC were the prognostic factors for prognosis of vulnerable stage of chronic heart failure(P<0.05).Conclusion Noninvasive hemodynamic indices such as LVEF,LCW and TFC are important prognostic factors of the vulnerable stage of heart failure patients,which may have predictive value for the prognosis of the vulnerable stage of heart failure and have reference significance for guiding the clinical treatment of patients wit h chronic heart failure.
