子宫背带式缝合术对剖宫产产后出血产妇术中及产后出血量的影响研究

摘　　要：目的探讨子宫背带式缝合术对剖宫产产后出血产妇术中及产后出血量的影响。方法30例行剖宫产分娩且伴有产后出血的产妇,以随机信封法分为对照组与观察组,每组15例。对照组采用子宫腔纱布填塞术治疗,观察组采用子宫背带式缝合术治疗。比较两组治疗效果、临床指标、生活质量及并发症发生情况。结果观察组治疗总有效率93.33%高于对照组的60.00%(P<0.05)。观察组手术用时(53.61±8.56)min、术中出血量(571.26±60.19)ml、术后24 h出血量(221.32±25.74)ml、住院时间(6.05±1.21)d均优于对照组的(62.17±10.25)min、(681.37±57.84)ml、(251.58±23.78)ml、(7.89±1.79)d(P<0.05)。观察组躯体功能、认知功能、情绪功能、角色功能、社会功能评分均高于对照组(P<0.05)。观察组并发症发生率13.33%低于对照组的46.67%(P<0.05)。结论以子宫背带式缝合术治疗剖宫产产后出血产妇,可减少患者术中及术后出血量,缩短手术用时及住院时间,同时还具有较高治疗安全性,利于促进患者术后恢复,临床借鉴价值较高,适宜推广借鉴。Objective To explore the effect of uterus strap suture on intraoperative and postpartum hemorrhage after cesarean section.Methods 30 parturients who delivered by cesarean section and accompanied by postpartum hemorrhage were divided into a control group and an observation group by random envelope method,with 15 cases in each group.The control group was treated with intrauterine gauze tamponade,and the observation group was treated with uterus strap suture.The therapeutic effect,clinical indicators,quality of life,and complications were compared between the two groups.Results The total effective rate of the observation group was 93.33%,which was higher than 60.00% of the control group(P<0.05).In the observation group,the operation time was(53.61±8.56)min,the intraoperative blood loss was(571.26±60.19)ml,the blood loss at 24 h postoperatively was(221.32±25.74)ml,and the hospital stay was(6.05±1.21)d,which were better than(62.17±10.25)min,(681.37±57.84)ml,(251.58±23.78)ml,and(7.89±1.79)d in the control group(P<0.05).The scores of physical function,cognitive function,emotional function,role function and social function in the observation group were higher than those in the control group(P<0.05).The complication rate of the observation group was 13.33%,which was lower than 46.67% of the control group(P<0.05).Conclusion In the treatment of women with postpartum hemorrhage after cesarean section,the use of uterus strap suture can reduce intraoperative and postoperative blood loss,shorten the operation time and hospital stay,and also has a high therapeutic safety,which is conducive to promoting postoperative recovery of patients,and has high clinical reference value,which is suitable for popularization and reference.
