三种口腔护理方法在颌间固定术后患者口腔护理效果研究

摘　　要：目的观察比较三种口腔护理方法在颌骨骨折行颌间固定术后患者中的口腔护理效果,以探寻更有效的口腔护理方法对颌间固定术后病人进行口腔护理。方法选择2022年7月-2023年12月在钦州市第二人民医院行颌间固定术的颌骨骨折病人102例,随机分为甲、乙、丙三组,每组各34例,甲组采用棉球擦拭法进行口腔护理,乙组采用注射器口腔冲洗法进行口腔护理,丙组采用输液式口腔冲洗+冲吸式小头软毛牙刷擦洗法进行口腔护理。干预1周,比较三种口腔护理方法对颌间固定术后病人的舒适度、口腔清洁度、牙龈指数(GI)、牙龈出血指数(GBI)、不良反应及并发症(误吞或误吸、口角损伤)等指标的优劣。结果在舒适度、口腔清洁度、牙龈指数(GI)、牙龈出血指数(GBI)评分方面,丙组评分均显著低于甲组与乙组(P<0.05)。在不良反应及并发症发生情况方面,丙组伤口感染发生率与甲组、乙组相比,降低更为显著,差异存在统计学意义(P<0.05)。结论输液式口腔冲洗+冲吸式小头软毛牙刷擦洗法在颌骨骨折颌间固定术后患者口腔护理中效果显著,值得临床推广和使用。Objective Observe and compare the effect of three oral care methods in the oral care of patients after intermaxillary fixation for jaw fracture,in order to explore more effective oral care methods for oral care of patients after intermaxillary fixation.Method 102 patients with jaw fracture who underwent intermaxillary fixation in our hospital from July 2022 to December 2023 were selected and randomly divided into three groups,A,B and C,with 34 cases in each group.Group A used the cotton ball wiping method to perform oral care,group B used the syringe oral rinse method to perform oral care,and group C used the infusional oral rinsing+rinsing and suctioning small-ended soft-bristled toothbrush scrubbing method to perform oral care.After 1 week of intervention,the advantages and disadvantages of the three oral care methods were compared in terms of patient comfort,oral cleanliness,gingival index(GI),gingival bleeding index(GBI),adverse reactions and the occurrence of complications(mis-swallowing or mis-absorption,and orofacial injuries)in the postoperative period of intermaxillary fixation.Result In terms of comfort,oral cleanliness,gingival index(GI),and gingival bleeding index(GBI)scores,the scores of group C were significantly lower than those of groups A and B(P<0.05).In terms of adverse reactions and complications,the incidence of wound infection in group C was more significantly reduced compared with groups A and B,and the difference was statistically significant(P<0.05).Conclusion Infusion oral rinsing+rinsing and suctioning small-headed soft-bristled toothbrush scrubbing method is effective in oral care of patients after intermaxillary fixation of jaw fracture,which is worthy of clinical promotion and use.
