摘 要:目的為新生兒早發型敗血症(early-onset sepsis,EOS)的有效管理和抗菌素需求之間取得平衡提供參考依據。方法回顧性分析2021年1月~12月本院收治之新生兒早發型敗血症,收集臨床表現、實驗室結果及母親高危因素等數據。比較現行之管理策略與用敗血症風險計算器分析後建議之策略。結果共收集157例,兩種策略在抗菌治療需求及治療天數上存在統計學差異(P<0.05)。安全性方面,一例被計算器評級無需抗菌之患兒出現病情轉差。結論兩種策略對本院新生兒早發型敗血症的處置各有優缺點。Objective To provide a reference for the balance between the effective management and antibiotic requirement in neonatal early-onset sepsis(EOS).Methods A retrospective analysis was performed on our 157 EOS patients from January 2021 to December 2021.Clinical manifestations,laboratory data,and maternal high-risk factors were collected.The data were analyzed using the Neonatal Early-Onset Sepsis Calculator,and compare the antibiotics requirement between two management protocol.Results Compared with two management protocol,there were statistical differences in antibiotics requirement and treatment days(P<0.05)for these 157 case.In terms of safety,a child who was graded to be clinically observed experienced deterioration during progress.Conclusion Both potocol have advantages and disadvantages for our EOS management.
