HIV-1感染者蛋白印迹带型特征与免疫功能相关性分析

摘  要:目的通过对HIV-1感染者蛋白印迹带型(WB)和CD4^(+)T淋巴细胞(CD4细胞)计数结果的分析,探讨不同WB带型特征与感染者免疫状态的相关性,从而为临床诊疗提供依据。方法纳入271例HIV-1型感染者,应用流式细胞术和免疫印迹法分别检测感染者静脉血中CD4细胞计数以及WB带型,分析不同年龄、性别与免疫缺陷程度、WB带型的关系,以及不同免疫缺陷程度与WB带型的关系。结果不同年龄组间的免疫缺陷程度差异有统计学意义(χ^(2)=28.65,P<0.001),而单个抗体条带分布差异无统计学意义(均P>0.05)。不同性别间仅p55抗体条带的分布差异具有统计学意义(χ^(2)=4.51,P<0.05),而免疫缺陷程度差异无统计学意义(P>0.05)。在不同免疫状态下,WB带型中p31、p55、p17抗体条带的分布差异有统计学意义(χ^(2)值分别为7.87、19.44、7.95,均P<0.05),HIV-1抗体全条带、gag基因编码全带、p55+p31缺失、p55+p17缺失的检出率差异有统计学意义(χ^(2)值分别为4.75、16.23、8.51、8.00,均P<0.05)。结论感染者的免疫状态会影响WB带型特征,其中p17抗体与p55抗体条带同时丢失可能与HIV感染程度有关。WB带型分析对治疗艾滋病的疗效评价和免疫重建监测可能有一定的帮助。Objective Through the analyses of the Western blot(WB)band type and CD4^(+)T lymphocyte(CD4 cells)count in patients with HIV-1 infection to explore the association between the characteristics of WB bands and the immune status of patients with HIV-1 infection,and to provide a basis for clinical diagnosis and treatment.Methods A total of 271 patients with HIV-1 infection were enrolled.The CD4 cell count and WB band type of the venous blood from the infected patients were detected by flow cytometry and immunoblotting,respectively.The relationship between different ages and gender,and the severity of immunodeficiency and WB band type were analyzed.Results The severity of immunodeficiency(χ^(2)=28.65,P<0.001),but not the distribution of WB bands,differed significantly among different age groups.However,the distribution of p55 band(χ^(2)=4.51,P<0.05),but not the severity of immunodeficiency,was significant different between genders.The distribution of the WB band types of p31,p55 and p17 varied greatly with immune status(χ^(2)=7.87,19.44,7.95,P<0.05 for all).Similarly,immune status significantly influenced the detection rates of the HIV-1 band,gag gene coding protein band,p55+p31 deficiency,and p55+p17 deficiency(χ^(2)=4.75,16.23,8.51,8.00,P<0.05 for all).Conclusions The immune status of infected individuals can affect the characteristics of WB bands.The simultaneous loss of p17 and p55 bands may be related to the severity of HIV infection.Analyses of WB bands would be helpful for evaluating the efficacy of AIDS treatment and monitoring the immune reconstitution.
