摘 要:经导管主动脉瓣置换术(transcatheter aortic valve replacement,TAVR)已经改变了严重主动脉瓣狭窄患者的治疗方法。然而,传导干扰的发生并没有随着时间的推移而显著减少,并且仍然是该治疗的主要并发症。此外,新发心房颤动是住院期间最常见的快速性心律失常,与更差的临床结果相关。然而,对于围手术期TAVR后心律失常事件的发生率和临床影响知之甚少。动态心电图(ambulatory electrocardiogram,AECG)监测最近已成为解决TAVR前后心律失常(缓慢性心律失常和快速性心律失常)复杂问题的工具。本综述概述了AECG监测在TAVR环境中的临床现状、临床意义以及发展前景,为主动脉瓣狭窄的治疗带来了新的时代。Transcatheter aortic valve replacement(TAVR)has changed the treatment of patients with severe aortic stenosis.However,the occurrence of conduction interference did not decrease significantly over time and it is still the main complication of the treatment.In addition,new onset atrial fibrillation was the most common tachyarrhythmia during hospitalization,which was associated with worse clinical outcomes.However,little is known about the incidence and clinical impact of arrhythmic events after perioperative TAVR.Recently,ambulatory electrocardiogram(AECG)monitoring has become a tool to solve the complex problem of arrhythmia(bradyarrhythmia and tachyarrhythmia)before and after TAVR.This review summarizes the status quo,clinical significance and future prospects of transcatheter aortic valve replacement(TAVR)in monitoring AECG in TAVR environment,which brings a new era for the treatment of aortic stenosis.
