摘  要:目的 探究钆塞酸二钠(Gd-EOB-DTPA)增强MR图像信号强度及影像组学评估肝功能的可行性。方法 分析105例肝硬化患者的资料,按Child-Pugh评分系统将患者分为Child-Pugh A级(62例)、Child-Pugh B级(38例)、Child-Pugh C级(5例)。测量患者的肝脏相对强化程度(RE)、肝胆期肝脏-脾信号比(SI肝/脾)、肝胆期肝脏-竖脊肌信号比(SI肝/肌)以及胆管信号强度对比率(SIR),并利用MaZda软件提取肝胆期图像影像组学特征,用Fisher、POE+ACC、MI 3种方法对提取的特征参数进行降维筛选,再行Spearman相关分析排除冗余参数。利用受试者工作特征(ROC)曲线定量评估各信号强度及影像组学特征的诊断效能,并基于这些参数构建多变量Logistic回归分析。结果 RE、SI_(肝/脾)、SI_(肝/肌)、SIR鉴别Child-Pugh A组与Child-Pugh B+C组的曲线下面积分别为0.805、0.740、0.630、0.703。筛选出的影像组学特征参数Perc.10%、S(0,5)Correlat、Teta4在鉴别Child-Pugh A组与Child-Pugh B+C组的曲线下面积分别为0.852、0.807、0.754。构建的多变量Logistic回归分析模型的曲线下面积为0.961,95%CI为0.929~0.993。结论 基于Gd-EOB-DTPA增强MR图像的各信号强度参数及影像组学特征可反映肝功能情况,且两者联合构建的模型能较好评估肝功能。Objective To investigate the feasibility of gadoxetic acid disodium(Gd-EOB-DTPA)enhanced MR image signal intensity and imaging histology for assessing liver function.Methods The data of 105 patients with liver cirrhosis were analyzed,and the patients were classified into Child-Pugh class A(62 patients),Child-Pugh class B(38 patients),and Child-Pugh class C(5 patients)according to the Child-Pugh scoring system.The relative degree of enhancement(RE)of the liver,the liver-spleen signal ratio(SIliver/spleen)in the hepatobiliary phase,the liver-vertical muscle signal ratio(SIliver/muscle)in the hepatobiliary phase and the bile duct signal intensity contrast ratio(SIR)of the patients were measured,and the imaging histological features of the images in the hepatobiliary phase were extracted using MaZda software,and the images were analysed by Fisher,POE+ACC,and MI three methods to downscale the extracted feature parameters,and then Spearman correlation analysis to exclude redundant parameters.The diagnostic efficacy of each signal intensity and imaging features was quantitatively evaluated by using the receiver operating characteristic curve(ROC)curve,and multivariate Logistic regression analysis was constructed based on these parameters.Results The areas under the curve of RE,SI_(liver/spleen),SI_(liver/muscle) and SIR were 0.805,0.740,0.630,and 0.703,respectively,to discriminate Child-Pugh A from ChildPugh B+C.The parameters of the imaging histological characteristics,Perc.10%,S(0,5)Correlat,and Teta4 were effective in discrimination between Child-Pugh A and Child-Pugh B+C groups.Child-Pugh A and Child-Pugh B+C groups were 0.852,0.807,and 0.754,respectively.The area under the curve of the constructed multivariate Logistic regression analysis model was 0.961,with 95%CIs ranging from 0.929 to 0.993.Conclusion Based on the Gd-EOB-DTPA-enhanced MR images of the various signal intensity parameters and imaging histological features,the liver function,and the model constructed by the combination of the two can better assess
