PCOS不孕患者促排卵期间血清抗苗勒管激素、抑制素B变化意义及与性激素的关系探讨

摘　　要：目的:探讨多囊卵巢综合征(PCOS)不孕患者促排卵期间血清抗苗勒管激素(AMH)、抑制素B(INHB)变化意义及与性激素的关系。方法:选取2021年12月—2022年12月瑞金市妇幼保健院收治的62例PCOS不孕患者设为观察组,同时选取同期在本院开展健康体检的健康女性62例设为对照组。观察组给予来曲唑联合人绒毛膜促性腺激素(hCG)促排卵治疗。比较两组基础INHB、AMH及性激素水平,比较观察组治疗前、治疗第4天及hCG注射日的血清INHB、AMH、促黄体生成素/促卵泡激素(LH/FSH)、孕酮(P)水平及窦卵泡计数(AFC)数量,采用Pearson分析PCOS不孕患者INHB、AMH水平和性激素的相关性。结果:与对照组相比较,观察组INHB、AMH、LH、LH/FSH、P水平及AFC数量均明显较高,FSH水平较低(P<0.05);随着时间的推移,观察组INHB、AMH、LH、LH/FSH、P水平及AFC数量呈逐渐下降的趋势,FSH呈增长趋势,且hCG注射日患者的INHB、AMH、FSH、LH、LH/FSH、P水平及AFC数量均明显优于治疗第4天和治疗前(P<0.05)。Pearson线性相关性分析结果显示,PCOS不孕患者的INHB水平和LH/FSH、P、AFC水平均呈正相关(P<0.05);且PCOS不孕患者的AMH水平也和LH/FSH、P、AFC呈正相关(P<0.05)。结论:PCOS不孕患者促排卵期间其血清INHB、AMH水平呈动态变化,同时和性激素有密切关联,其中患者的血清INHB、AMH水平与LH/FSH、P、AFC均呈正相关。Objective:To explore the significance of changes in serum anti Mullerian hormone(AMH)and inhibin B(INHB)levels during ovulation induction in infertile patients with polycystic ovary syndrome(PCOS)and their relationship with sex hormones.Method:A total of 62 patients with PCOS infertility admitted to Ruijin Maternal and Child Health Care Hospital from December 2021 to December 2022 were selected as the observation group,while 62 healthy women who underwent health examinations in our hospital during the same period were selected as the control group.The observation group was treated with Letrozole combined with Human Chorionic Gonadotropin(hCG)to promote ovulation.The baseline levels of INHB,AMH,and sex hormones between two groups were compared.the serum levels of INHB,AMH,luteinizing hormone/follicle stimulating hormone(LH/FSH),progesterone(P),and antral follicle count(AFC)before treatment,on the 4~(th)day of treatment,and on the day of hCG injection in the observation group were compared.The correlation between INHB,AMH levels,and sex hormones in PCOS infertility patients were analyzed by Pearson.Result:Compared with the control group,levels of INHB,AMH,LH,LH/FSH,P and AFC in the observation group were significantly higher,the level of FSH was lower(P<0.05);as time passed,the levels of INHB,AMH,LH,LH/FSH,P and AFC in the observation group showed a gradually decreasing trend,the FSH showed a increasing trend,and the levels of INHB,AMH,FSH,LH,LH/FSH,P and AFC in patients on the day of hCG injection were significantly better than those on the 4th day of treatment and before treatment(P<0.05).The Pearson linear correlation analysis results showed that the INHB level in PCOS infertility patients were positively correlated with LH/FSH,P and AFC levels(P<0.05);and the AMH level in PCOS infertility patients were also positively correlated with LH/FSH,P and AFC levels(P<0.05).Conclusion:The serum levels of INHB and AMH in infertile patients with PCOS show dynamic changes during ovulation induction,and are closely related
