完带汤联合聚焦超声治疗脾虚湿盛型宫颈上皮内瘤变的效果分析

摘　　要：目的观察完带汤联合聚焦超声治疗宫颈上皮内瘤变的疗效。方法选取2021年6月—2022年5月于临沂市中心医院行宫颈活组织检查确诊为高危HPV感染合并宫颈上皮内瘤变的患者60例,采用随机数表法分为对照组(30例)和试验组(30例),对照组予单纯海扶高强度聚焦超声治疗,试验组予完带汤同时配合海扶高强度聚焦超声治疗。治疗前后对两组患者的临床疗效、阴道镜评分、HPV转阴率、中医证候主症评分、中医证候次症评分及血液免疫学指标(CD4^(+)T、CD8^(+)T、CD4^(+)T/CD8^(+)T)进行分析。结果治疗后,试验组疗效高于对照组,差异有统计学意义(P>0.05);试验组阴道镜评分高于对照组,差异有统计学意义(P<0.05);治疗后,HPV转阴率试验组高于对照组,差异有统计学意义(P>0.05);治疗后两组中医证候评分均低于治疗前,试验组次症评分低于对照组,差异有统计学意义(P<0.05)。治疗后,两组淋巴细胞亚群比例CD4^(+)T、CD4^(+)T/CD8^(+)T较治疗前均有明显改善,且试验组高于对照组,差异有统计学意义(P<0.05)。结论完带汤联合高强度聚焦超声对CIN患者有较好的临床疗效,能改善患者的中医证候评分,调节患者的机体免疫平衡,值得临床研究应用。Objective To investigate and observe the therapeuticeffect of Wandai decoction combined with focused trasound in patients with cervical intraepithelial neoplasia and influence for body immune.Methods A total of 60 patients diagnosed with cervical intraepithelial neoplasia by cervical biopsy in Linyi Central Hospital from June 2021 to May 2022 were selected and randomly divided into control group(30 cases)and trial group(30 cases).The control group was treated with Haifu highintensity focused ultrasound alone,and the trial group was treated with Wandai decoction combined with Haifu high-intensity focused ultrasound.After treatment,the curative effect,the negative conversion rates of human papillomavirus(HPV),TCM syndrome score,CD4^(+)T,CD8^(+)T,CD4^(+)T/CD8^(+)T were analyzed.the main symptoms scoreof TCM syndrome,the secondary symptoms scoreof TCM syndrome,and the blood immunological indexes(CD4^(+)T,CD4^(+)T,CD4^(+)T/CD8^(+)T)were analyzed.Results The curative effect in the experimental group was higher than that in the control group(P>0.05).After treatment,the D-value of Reid’s Colposcopy index in the trial group was higher than that in the control group(P<0.05),and the negative conversion rate of HPV in the trial group was higher than that in the control group at 3 months(P>0.05),After treatment,the scores of TCM syndromes in both groups were lower than those before treatment(P<0.05).and the scores of secondary symptoms in the experimental group were lower than those in the control group(P>0.05).After treatment,the lymphocyte ratio of CD4^(+)T and CD4^(+)T/CD8^(+)T in both groups were significantly improved,and the experimental group was superior to the control group(P<0.05).Conclusion Wandai decoction combined with high intensity focused ultrasound has a better therapeutic effect on CIN patients,which can improve the TCM syndromes and balance the immune status of the patients,it is worthy of clinical application.
