摘 要:目的检测吸烟者及慢性阻塞性肺疾病(COPD)患者外周血细胞毒性T淋巴细胞相关抗原-4(CTLA4)/B7及Treg细胞的表达,观察免疫指标在COPD外周血中的变化及临床意义。方法选择8名健康志愿者为A组(正常对照组)、8名吸烟但肺功能正常者为B组、8名轻中度慢性阻塞性肺疾病患者为C组,收集3组研究对象的外周血标本,采用流式细胞术分析方法检测各组外周血单个核细胞CD11c^(+)树突状细胞(DCs)的比例、CD11c^(+)DCs表面CD80及CD86的表达,Treg细胞(CD4^(+)CD25^(+)FOXP3^(+))及CD4^(+)CD25^(+)CTLA4^(+)的表达。结果与A组比较,B组及C组的CD11^(+)DCs、CD80(B7-1)和CD86(B7-2)表达均显著增多,差异均有统计学意义(均P<0.05)。在Treg细胞表达上,与A组相比,B组及C组的Treg细胞减少(均P<0.05),而B组与C组间比较无统计学意义(P>0.05)。与A组相比,B组及C组的CTLA4^(+)细胞表达增多,差异均有统计学意义(均P<0.05)。结论吸烟者及COPD患者外周血中CTLA4/B7呈高表达,Treg细胞比例减少,提示吸烟所致免疫细胞异常可能参与COPD全身炎症进展过程。Objective To investigate the expression of cytotoxic T lymphocyte-associated antigen-4(CTLA4)/B7 and Treg cells in the peripheral blood of smokers and patients with chronic obstructive pulmonary disease(COPD),and to examine the changes and clinical significance of immune indicators in COPD.Methods This study recruited 8 healthy volunteers as the normal control group(Group A),8 smokers with normal lung function(Group B),and 8 patients with mild to moderate COPD(Group C).Flow cytometry analysis was utilized to assess the proportion of CD11c^(+)dendritic cells(DCs)in peripheral blood mononuclear cells,the surface expression of CD80 and CD86 on CD11c^(+)DCs,as well as Treg cells(CD4^(+)CD25^(+)FOXP3^(+))and CD4^(+)CD25^(+)CTLA4^(+)cells.Results In comparison to group A,the expression of CD11^(+)DCs,CD80(B7-1),and CD86(B7-2)significantly increased in groups B and C,with statistically significant differences(all P<0.05).Regarding Treg cells,the number decreased in groups B and C compared to group A,with statistically significant differences(all P<0.05),but no statistical variance between groups B and C(P>0.05).Additionally,the expression of CTLA4^(+)cells in groups B and C increased significantly compared to group A(all P<0.05).Conclusion High expression of CTLA4/B7 and reduced Treg cell proportions in the peripheral blood of smokers and COPD patients suggest a potential link between smoking-induced immune cell abnormalities and systemic inflammation in COPD.
