摘  要:目的 评价5G远程控制骨科机器人辅助经皮螺钉内固定治疗骨盆髋臼骨折手术的安全性和有效性。方法 回顾性分析2023年3月—8月由北京积水潭医院创伤骨科医师在国家骨科医学中心5G+骨科远程手术中心,通过控制河北省沧州中西医结合医院骨科机器人共治疗骨盆髋臼骨折患者33例,男性21例,女性12例;年龄18~78岁,平均50.1岁。致伤原因:道路交通伤13例,高处坠落伤15例,水平地面摔伤3例,重物砸伤2例。其中骨盆骨折19例,髋臼骨折14例。共置入通道螺钉76枚。评价指标包括螺钉置入准确性、术中不良事件发生率、螺钉规划时间和螺钉置入时间以及术后1周内并发症发生率。结果置钉位置评价优69枚(90.8%),良7枚(9.2%),无螺钉穿透皮质骨。螺钉采集图像后至规划完成时间为3~8 min,平均5.3 min;螺钉置入时间2~7 min,平均3.5 min。手术操作中未发生任何不良事件。患者术后1周内均未出现伤口感染、血肿、内固定松动或断裂、术后再骨折、血管神经损伤等并发症。患者均对手术效果表示满意。结论 5G远程控制骨科机器人技术通过高速、低延迟的5G网络连接手术室与远程控制中心,允许外科医师在不同地点精确操控机器人进行手术操作,在提高手术精度和安全性方面具有明显优势。Objective To evaluate the effectiveness and safety of 5G remote control orthopedic robot-assis-ted percutaneous screw fixation for the treatment of pelvic and acetabular fractures.Methods From Mar.to Aug.2023,33 patients with pelvic and acetabular fractures who received robot-assisted percutaneous screw fixation were included.All the channel screws,76 in total,were implanted by an orthopedic robot that was remotely controlled by trauma surgeons from Beijing Jishuitan Hospital at the 5G+Orthopedic Remote Surgery Center of the National Ortho-pedic Medical Center.Of the patients,there were 21 males and 12 females,with the ages ranging from 18 to 78 years,mean 50.1 years.Injury mechanisms were road traffic injuries in 13 cases,falls from heights in 15,ground-lev-el falls in 3,and hit by heavy objects in 2.Among them,there were 19 pelvic fractures and 14 acetabular fractures.The screw insertion accuracy,incidence of intraoperative adverse events,screw planning time and actual insertion time,and incidence of complications within one week after surgery were assessed.Results Position of the implanted channel screws was excellent in 69 cases(90.8%)and good in 7 cases(9.2%),with no screw penetrating the cor-tical bone.The time from screw image acquisition to finish of insertion design was 3-8(mean 5.3)min,and the screw insertion time was 2-7(3.5)min.No adverse event occurred during the surgical procedure.Within one week after surgery,none of the patients experienced any complications such as wound infection,hematoma,loose or broken internal fixation,postoperative re-fractures,or vascular and nerve injuries.All the patients were satisfied with the sur-gical results.Conclusion The application of 5G remote control orthopedic robot technology in percutaneous screw insertion surgery for pelvic and acetabular fractures connects the operating room and the remote control center through a high-speed,low-latency 5G network,allowing surgeons to precisely control robots for surgical operations at different locations,which has signific
