联合扩散峰度成像及定量磁化率成像评价脑瘫伴视觉障碍患儿视通路微观结构损害的初步研究

摘  要:目的:通过联合应用MRI扩散峰度成像(DKI)和定量磁化率成像(QSM),探讨DKI和QSM评价脑性瘫痪(脑瘫)伴视觉障碍患儿视通路微观结构改变的价值。方法:对30例脑瘫伴视觉障碍患儿(患儿组)及30例健康儿童(对照组)分别行常规MRI及DKI、QSM扫描,测量和对比2组视神经、外侧膝状体、视放射和视皮质区ROI的平均峰度(MK)值、部分各向异性(FA)值、平均扩散率(MD)值及磁化率的差异。结果:2组在视神经、外侧膝状体、视放射和视皮质区域测量的MK值差异均有统计学意义(均P<0.05);2组在视神经、外侧膝状体和视放射区域测量的FA、MD值差异均有统计学意义(均P<0.05);2组在外侧膝状体、视放射区域测量的磁化率差异均有统计学意义(均P<0.05)。结论:联合DKI和QSM能定量评估脑瘫伴视觉障碍患儿视通路各区域中微观结构的变化,从而为无创性评估脑瘫患儿视通路损害提供重要的影像学依据。Objective:To investigate the value of diffusion kurtosis imaging(DKI)and quantitative susceptibility mapping(QSM)in evaluating the changes in the microstructure of visual pathway in cerebral palsy children with visual impairment.Methods:30 cerebral palsy children with visual impairment(the observation group)and 30 healthy volunteers(the control group)underwent conventional MRI,DKI and QSM scan.The mean kurtosis(MK),partial fractional anisotropy(FA),mean diffusivity(MD)and magnetic susceptibility of each ROI in the optic nerve,lateral geniculate body,optic radiation and visual cortex were measured and compared between the two groups.Results:There were statistically significant differences in MK values in the optic nerve,lateral geniculate body,optic radiation and visual cortex between the two groups(all P<0.05).There were statistically significant differences in FA and MD values in the optic nerve,lateral geniculate body and optic radiation between the two groups(all P<0.05).And there were statistically significant differences in magnetic susceptibility in the lateral geniculate body,optic radiation between the two groups(all P<0.05).Conclusions:The combination of DKI and QSM can quantitatively evaluate the microstructure changes of visual pathway in cerebral palsy children with visual impairment,which provides an important imaging basis for noninvasive evaluation of visual pathway damage in cerebral palsy children.
