双胎妊娠宫颈机能不全患者行腹腔镜下宫颈内口环扎术疗效分析

摘　　要：目的 探究双胎妊娠宫颈机能不全孕早期患者在腹腔镜下行改良宫颈内口环扎术的疗效。方法 选择2019年1月~2022年12月我院收治的双胎妊娠宫颈功能不全患者12例为观察组,采用腹腔镜下改良宫颈内口环扎术治疗;同时选择双胎妊娠宫颈机能不全行经阴道宫颈环扎术患者15例作为对照组;比较两组患者的妊娠结局和新生儿情况。结果 除观察组患者宫颈管长度大于对照组(P<0.05)之外,两组患者的流产率、≤32周早产率、<37周早产率、分娩孕周数以及新生儿5min Apgar评分<8分发生率、低体重儿率、转NICU率比较,差异均无统计学意义(P>0.05)。结论 对双胎妊娠宫颈功能不全患者采用腹腔镜下改良宫颈内口环扎术与传统经阴道宫颈内口环扎术疗效相同,临床可根据患者实际情况确定手术方式,但术中麻醉用药对胎儿发育情况的影响有待进一步研究。Objective To investigate and analyze the efficacy of laparoscopic cervicoisthmic cerclage in the treatment of cervical insufficiency and the clinical observation of patients with cervical insufficiency in twin pregnancies.Methods From January 2019 to December 2022,a total of 12 patients with cervical insufficiency of twin pregnancy were retrospectively analyzed.As the observation group of this study,15 patients with cervical incompetence of twin pregnancy in our hospital were selected as the control group,and the pregnancy outcome and neonatal outcome were observed.Re-sults The cervical canal length in the observation group was longer than that in the control group(P<0.05).There was no significant difference between the two groups in abortion rate,premature delivery rate of≤32 weeks,premature deliv-ery rate of<37 weeks,gestational age of delivery,Apgar score of 5 minutes<8 points,low birth weight infant rate and neonatal intensive care unit transfer rate(P>0.05).Conclusion The improved laparoscopic cervical cerclage or trans-vaginal cervical cerclage has the same effect on the patients with cervical incompetence in twin pregnancy,and the spe-cific operation method can be determined according to the actual situation of the patients,but the anesthetic drugs during the operation need to be further studied.
