摘 要:目的探讨2型糖尿病(T2DM)并发脑梗死患者血清胱抑素C(Cys-C)水平与脑梗死病情严重程度的相关性。方法回顾性抽取2020年5月至2023年5月西安市人民医院收治的93例T2DM并发脑梗死患者作为T2DM合并脑梗死组,采用美国国立卫生院卒中量表(NIHSS)评分评估病情,其中轻度43例,中度30例,重度20例;另抽取同期收治的60例单纯T2DM患者作为单纯T2DM组,60例体检健康者作为健康组。比较三组一般资料,比较三组及T2DM合并脑梗死组中不同病情者血清Cys-C水平、Barthel指数(BI)、改良Rankin量表(mRs)评分,应用Pearson相关性分析探讨T2DM并发脑梗死患者血清Cys-C水平与BI、mRS评分、NIHSS评分的相关性。结果三组性别、年龄、体质指数、T2DM病程、吸烟、饮酒情况比较,差异未见统计学意义(P>0.05)。T2DM并发脑梗死组Cys-C水平高于单纯T2DM组、健康组(P<0.05),T2DM并发脑梗死组BI、mRS评分与单纯T2DM组和健康组比较,差异有统计学意义(P<0.05)。T2DM合并脑梗死组中不同病情者Cys-C、BI、mRS水平比较,差异有统计学意义(P<0.05)。Pearson相关分析结果显示,T2DM并发脑梗死患者血清Cys-C水平与NIHSS、mRS评分呈正相关,与BI指数呈负相关(r=0.70、0.72、-0.45,P均<0.05)。结论2型糖尿病并发脑梗死患者血清Cys-C水平处于较高水平,且与脑梗死病情严重程度密切相关。Objective To investigate the correlation between the level of serum cystatin C(Cys-C)and severity of cerebral infarction in patients with type 2 diabetes mellitus(T2DM)complicated by cerebral infarction.Methods A total of 93 patients with T2DM complicated by cerebral infarction admitted to Xi’an People’s Hospital from May 2020 to May 2023 were retrospectively selected as the T2DM complicated by cerebral infarction group.The American National Institutes of Health stroke scale(NIHSS)score was used to evaluate the disease condition,including 43 cases of mild,30 cases of moderate,and 20 cases of severe.In addition,60 patients with simple T2DM hospitalized during the same period were selected as the T2DM group;and 60 healthy patients who underwent physical examination were selected as the healthy group during the same period.The general data,level of serum Cys-C,Barthel index(BI)and modified Rankin scale(mRs)scores were compared among the three groups and among patients with different disease conditions in the T2DM complicated by cerebral infarction group.Pearson correlation analysis was used to investigate the correlations of serum Cys-C level with BI,mRS score and NIHSS score in patients with T2DM complicated by cerebral infarction.Results There were no significant differences in gender,age,body mass index,course of T2DM,smoking,and alcohol consumption among the three groups(P>0.05).The level of Cys-C in the T2DM complicated by cerebral infarction group was higher than that in the T2DM group and the healthy group(P<0.05).The BI and mRS scores of the T2DM complicated by cerebral infarction group had significant differences in comparison with the T2DM group and the healthy group(P<0.05).There were significant differences in levels of Cys-C,BI and mRS among patients with different disease conditions in the T2DM complicated by cerebral infarction group(P<0.05).The results of Pearson correlation analysis showed that the level of serum Cys-C in patients with T2DM complicated by cerebral infarction was positively corre
