摘　　要：目的探讨淀粉样蛋白A对成年人病毒性脑炎的诊断价值。方法本研究为病例对照研究,选取2021年12月—2022年12月在新疆医科大学第二附属医院住院的成年病毒性脑炎患者61例为病例组,选择同期在该院住院的非炎症性中枢系统疾病患者50例为对照组,收集患者的临床资料。采用多因素Logistic回归分析探讨成年人病毒性脑炎的影响因素,采用ROC曲线分析血清淀粉样蛋白A诊断成年人病毒性脑炎的价值。结果病例组C反应蛋白、血清淀粉样蛋白A高于对照组(P<0.05)。多因素Logistic回归分析结果显示,血清淀粉样蛋白A是成年人病毒性脑炎的独立影响因素〔OR=1.405,95%CI(1.225~1.612)〕。ROC曲线分析结果显示,血清淀粉样蛋白A诊断成年人病毒性脑炎的AUC为0.959〔95%CI(0.920~0.997)〕,最佳截断值为4.10 mg/L,灵敏度为96.7%,特异度为88.0%,约登指数为84.7%。结论血清淀粉样蛋白A是成年人病毒性脑炎的独立影响因素,其对成年人病毒性脑炎有较高的诊断价值。Objective To investigate the diagnostic value of amyloid A for adult viral encephalitis.Methods This study was a case-control study.Sixty-one adult patients with viral encephalitis admitted to the Second Affiliated Hospital of Xinjiang Medical University from December 2021 to December 2022 were selected as the case group,50 patients with noninflammatory central nervous system diseases admitted to the hospital during the same period were selected as the control group.Clinical data of the patients were collected.Multivariate Logistic regression analysis was used to explore the influencing factors of adult viral encephalitis.ROC curve was used to explore the diagnostic value of serum amyloid A for adult viral encephalitis.Results C-reactive protein and serum amyloid A in case group were higher than those in control group(P<0.05).Multivariate Logistic regression analysis showed that serum amyloid A was the independent influencing factor of viral encephalitis[OR=1.405,95%CI(1.225-1.612)].ROC curve analysis showed that the AUC of the serum amyloid A for diagnosing adult viral encephalitis was 0.959[95%CI(0.920-0.997)],the best cutoff value was 4.10 mg/L,the sensitivity was 96.7%,the specificity was 88.0%,and Youden index was 84.7%.Conclusion Serum amyloid A is an independent influencing factor of adult viral encephalitis,and it has a high diagnostic value for adult viral encephalitis.
