摘　　要：目的探讨经皮椎间孔镜脊柱系统(TESSYS)对腰椎间盘突出症(LDH)合并神经根管狭窄患者炎症反应的影响。方法使用随机排列表法将2018年6月-2023年6月期间灌云博爱医院收治的67例LDH合并神经根管狭窄患者分成试验组(n=33)和参照组(n=34)。参照组予以经椎间孔入路腰椎椎体间融合术(TLIF)治疗,试验组予以TESSYS技术治疗,观察比较研究对象炎症反应程度、术后疼痛程度、腰椎功能恢复情况以及治疗效果。结果术前试验组肿瘤坏死因子-α(TNF-α)、超敏C反应蛋白(hs-CRP)水平与参照组对比均无明显差异(P>0.05),术后试验组TNF-α、hs-CRP水平均明显低于参照组(P<0.05);试验组优良率(90.91%)显著比参照组(70.59%)高(P<0.05);术前试验组视觉模拟(VAS)评分、日本骨科协会评估治疗分数(JOA)评分与参照组对比均无明显差异(P>0.05),术后试验组VAS评分与参照组对比均明显降低,且JOA评分明显升高(P<0.05)。结论采用TESSYS治疗LDH合并神经根管狭窄患者,疗效甚佳,能够降低患者炎症反应程度,缓解术后疼痛水平,对腰椎功能恢复具有促进作用,值得临床推广。Objective To investigate the effect of percutaneous intervertebral scintigraphic spinal system(TESSYS)on inflammatory response in patients with lumbar disc herniation(LDH)combined with radicular stenosis.Methods Sixty-seven patients with LDH combined with radiculopathic stenosis admitted to our hospital during the period June 2018 to June 2023 were divided into a experimental group(n=33)and a reference group(n=34)using a randomized permutation table approach.The reference group was given transforaminal approach lumbar interbody fusion(TLIF),and the experimental group was given TESSYS technique to observe and compare the degree of inflammatory response,postoperative pain,recovery of lumbar spine function,and treatment effect of the study subjects.Results There was no difference in the levels of tumor necrosis factor-α(TNF-α)and ultrasensitive C-reactive protein(hs-CRP)in the experimental group compared with the reference group before surgery(P>0.05),and the levels of TNF-αand hs-CRP in the experimental group were significantly lower compared with the reference group after surgery(P<0.05);the good rate of the experimental group(90.91%)was significantly higher than that of the reference group(70.59%)(P<0.05);the preoperative experimental group was given TESSYS technique to observe and compare the degree of inflammatory reaction,the degree of pain,the recovery of lumbar spine function and the treatment effect of the study subjects(P>0.05);preoperative visual analog scores(VAS)and Japanese Orthopedic Association Assessment Treatment Score(JOA)scores of the experimental group were not different from those of the reference group(P>0.05),and postoperative VAS scores of the experimental group were significantly lower and JOA scores were significantly higher compared with those of the reference group(P<0.05).Conclusion The use of TESSYS in the treatment of patients with LDH combined with radicular stenosis has excellent efficacy,can reduce the degree of inflammatory response of patients,alleviate the level of postoperat
