基于主动健康指数的慢性病精准管理模式对血液透析患者液体摄入依从性、自我效能的研究

摘　　要：目的基于主动健康指数探讨慢性病精准管理模式对血液透析患者液体摄入依从性、自我效能的研究。方法选取2021年1月-2022年1月江苏省省级机关医院收治的96例血液透析患者作为研究对象。按照随机数字方法分为对照组及观察组,每组均48例,对照组采用常规护理,观察组实施基于主动健康指数的慢性病精准管理,观察两组患者干预前后液体摄入依从性、自我效能、营养及实验室指标、透析低血压发生情况。结果干预后,与对照组相比,观察组患者摄入依从性评分、CDSES评分及PAB、A1b均升高,血磷表达水平、透析低血压的发生率均降低(P<0.05)。结论基于主动健康指数的慢性病精准管理可显著提高患者液体摄入依从性、自我效能,并可增加机体营养状态的同时提高心理韧性,值得推荐。Objective Based on active health index,to investigate the effect of chronic disease precision management model on fluid intake compliance and self-efficacy of hemodialysis patients.Methods 96 patients with hemodialysis treated in our hospital from January 2021 to January 2022 were selected as the study objects.According to random number method,they were divided into control group and observation group,with 48 cases.The control group received routine nursing,and the observation group received accurate management of chronic diseases based on active health index.The fluid intake compliance,self-efficacy,nutrition and laboratory indexes,dialysis hypotension of patients before and after intervention were observed in both groups.Results After intervention,compared with the control group,the intake compliance scores,CDSES scores,PAB and Alb were increased in observation group,and the blood phosphorus expression level and the incidence of dialysis hypotension were decreased(P<0.05).Conclusion Accurate management of chronic diseases based on active health index can significantly improve the compliance of fluid intake,self-efficacy,increase the nutritional status of the body and reduce the psychological resilience,which is worthy of recommendation.
