终末期肾病患者首次血液透析治疗时的心理负担现状调查及其影响因素分析

摘　　要：目的调查分析终末期肾病患者首次血液透析治疗时的心理负担现状及其影响因素。方法使用自我感受负担量表对2022年8月-2023年9月在贵州省锦屏县人民医院治疗的100例首次血液透析治疗的终末期肾病患者进行问卷调查,分析自我负担影响因素对透析患者的影响。结果患者的SPBS平均分为(29.76±5.40)分,呈现中度负担状态;采用因素方差对患者不同人口学特征分析,发现不同年龄、职业状况、医疗费用支付方式、家庭人均月收入、有无并发症、透析频次、照护者健康状况SPBS得分差异有统计学意义(P<0.05);而多元线性回归分析显示年龄、并发症、医疗费用支付方式以及家庭收入情况是影响患者心理负担水平的因素(P<0.05)。结论在终末期肾病患者首次选择透析治疗时,医护人员可以结合医保费用、年龄、家庭状况和并发症因素,向患者提供个性化的心理支持,以此增强其治疗配合度,从而降低医疗风险。Objective To investigate the current psychological burden of end-stage renal disease patients during their first hemodialysis treatment and analyze its influencing factors.Method A questionnaire survey was conducted on 100 end-stage renal disease patients who underwent initial hemodialysis treatment in our hospital from August 2022 to September 2023 using the Self Perceived Burden Scale.The survey was analyzed to understand the impact of factors affecting patient self burden on dialysis patients.Result The total average SPBS score of the patients was(29.76±5.40)points,showing a moderate burden state;using factor variance analysis to analyze different demographic characteristics of patients,it was found that there were statistically significant differences(P<0.05)in age,occupational status,medical expense payment methods,per capita monthly income of families,presence or absence of complications,dialysis frequency,and SPBS scores of caregiver health status;multiple linear regression analysis showed that age,complications,payment methods for medical expenses,and family income were factors that affected the psychological burden level of patients(P<0.05).Conc lusion When end-stage renal disease patients choose dialysis treatment for the first time,medical staff can provide personalized psychological support to patients based on medical insurance costs,age,family status,and complications,thereby enhancing their treatment cooperation and reducing medical risks.
