摘 要:目的分析成人急性髓系白血病(AML)化疗患者院内感染危险因素及护理策略。方法选择2021年4月-2023年3月赣南医学院第一附属医院145例成人AML化疗患者为观察对象,统计院内感染发生情况,根据患者是否发生院内感染分为院内感染组、非院内感染组,收集并比较两组资料,分析成人AML化疗患者院内感染危险因素及护理策略。结果该研究145例成人AML化疗患者中,91例发生院内感染,院内感染发生率为62.76%(91/145);院内感染组、非院内感染组的性别、年龄、体重指数、文化程度、住院时间、化疗阶段、使用糖皮质激素、使用免疫抑制剂、血小板、白蛋白比较,差异无统计学意义(P>0.05);院内感染组化疗强度大的占比高于非院内感染组,中性粒细胞、血红蛋白低于非院内感染组,差异有统计学意义(P<0.05);经Logistic回归分析结果显示,化疗强度大、中性粒细胞降低、血红蛋白降低是成人AML化疗患者院内感染危险因素(OR>1,P<0.05)。结论成人AML化疗患者医院感染发生风险较高,与化疗强度大、中性粒细胞降低、血红蛋白降低有关。Objective To analyze the risk factors and nursing strategies for hospital acquired infections in adult acute myeloid leukemia(AML)patients undergoing chemotherapy.Method 145 adult AML chemotherapy patients in our hospital from April 2021 to March 2023 were selected,and the incidence of hospital acquired infections was statistically analyzed.Patients were divided into hospital acquired infection group and non hospital acquired infection group based on whether they had hospital acquired infections.Data were collected and compared between the two groups,and the risk factors and nursing strategies for hospital acquired infections in adult AML chemotherapy patients were analyzed.Result Among 145 adult AML chemotherapy patients in this study,91 cases developed hospital acquired infections,with a hospital acquired infection rate of 62.76%(91/145);there was no statistically significant difference in gender,age,body mass index,educational level,length of hospital stay,chemotherapy stage,use of glucocorticoids,use of immunosuppressants,platelets,and albumin between the hospital infection group and the non hospital infection group(P>0.05);the proportion of patients with high chemotherapy intensity in the hospital infection group was higher than that in the non hospital infection group,and neutrophils and hemoglobin were lower than those in the non hospital infection group,with statistical significance(P<0.05);the results of Logistic regression analysis showed that high chemotherapy intensity,decreased neutrophils,and decreased hemoglobin were risk factors for hospital acquired infections in adult AML chemotherapy patients(OR>1,P<0.05).Conclusion Adult AML chemotherapy patients have a higher risk of hospital acquired infections,which is related to high chemotherapy intensity,decreased neutrophils,and decreased hemoglobin.
