摘 要:目的分析妊娠期肝内胆汁淤积症(ICP)患者血清总胆汁酸(TBA)、天冬氨酸转氨酶(AST)表达与胎儿生长发育的关系。方法前瞻性研究。抽取2021年2月至2022年1月周口市中心医院收治的ICP患者96例作为ICP组,并以1∶1配比抽取同期健康孕妇96例作为健康组。检测并比较两组TBA、AST水平,比较两组胎儿生长发育情况及不良妊娠结局,采用Pearson分析TBA、AST表达与胎儿生长发育的相关性。结果ICP组TBA、AST水平高于健康组(P<0.05)。ICP组胎儿体质量、胎儿身高、Apgar评分低于健康组(P<0.05);ICP组羊水污染、早产、胎儿窘迫、胎儿窒息发生率[19.79%(19/96)、27.08%(26/96)、20.83%(20/96)、14.58%(14/96)]高于健康组[0.00%(0/96)、6.25%(6/96)、5.21%(5/96)、2.08%(2/96)],P<0.05。Pearson分析结果显示,TBA表达与胎儿体质量(r=-0.63)、胎儿身高(r=-0.57)、Apgar评分(r=-0.69)呈负相关(P<0.05),AST表达与胎儿体质量(r=-0.56)、胎儿身高(r=-0.44)、Apgar评分(r=-0.62)呈负相关(P<0.05)。结论ICP患者血清TBA、AST水平与胎儿生长发育和妊娠结局存在相关性,临床可实时检测TBA、AST水平,为诊断ICP和判断预后提供参考依据。Objective To analyze the correlation of the expression of serum total bile acid(TBA)and aspartate transaminase(AST)with fetal growth and development in patients with intrahepatic cholestasis of pregnancy(ICP).Methods Prospective studies were adopted in this study.A total of 96 patients with ICP treated in Zhoukou Central Hospital from February 2021 to January 2022 were selected as the ICP group,and 96 healthy pregnant women hospitalized during the same period were selected as the healthy group in a 1∶1 ratio.The levels of TBA and AST in the two groups were detected.The levels of TBA and AST,fetal growth and development,adverse pregnancy outcomes were compared between the two groups.The correlation of the expression of TBA and AST with fetal growth and development was analyzed by Pearson analysis.Results The levels of TBA and AST in the ICP group were higher than those in the healthy group(P<0.05).The fetal weight,fetal height and Apgar score in the ICP group were lower than those in the healthy group(P<0.05).The incidence of amniotic fluid pollution,preterm birth,fetal distress and fetal asphyxia in the ICP group were 19.79%(19/96),27.08%(26/96),20.83%(20/96),and 14.58%(14/96),respectively,which were correspondingly higher than the 0.00%(0/96),6.25%(6/96),5.21%(5/96)and 2.08%(2/96)in the healthy group(P<0.05).Results of Pearson analysis showed that the expression of TBA was negatively correlated with fetal weight(r=-0.63),fetal height(r=-0.57),and Apgar score(r=-0.69);while the expression of AST was negatively correlated with fetal weight(r=-0.56),fetal height(r=-0.44),and Apgar score(r=-0.62),P<0.05.Conclusions The levels of serum TBA and AST have relationships with fetal growth and pregnancy outcome of ICP patients.Clinical real-time detection of levels of TBA and AST can provide reference for diagnosing ICP and predicting prognosis.
