摘　　要：目的:通过检测T淋巴细胞亚群CD3^(+)、CD4^(+)、CD8^(+)及CD4^(+)/CD8^(+)在髋关节一过性滑膜炎儿童外周血中的表达水平,探讨其在疾病诊治、严重程度划分等方面的临床意义。方法:以2019年4月—2021年12月长春市儿童医院收治的130例患有髋关节一过性滑膜炎的儿童为观察组,同期在该医院进行健康体检的30名正常儿童为对照组。分析两组末梢血T淋巴细胞亚群数量,利用P<0.05的检测指标绘制受试者工作特征曲线(ROC曲线),分析其诊断效率。将具有髋关节彩超结果阳性的55例患儿外周血T淋巴细胞亚群与关节囊前壁厚径、关节腔积液深度进行相关性分析。结果:观察组总T淋巴细胞CD3^(+)绝对计数、辅助/诱导T淋巴细胞CD4^(+)绝对计数、抑制/杀伤T淋巴细胞CD8^(+)绝对计数明显低于对照组,差异均有统计学意义(Z值分别为-3.296、-3.097、-3.139,P<0.05)。观察组CD3^(+)、CD4^(+)、CD8^(+)绝对计数的ROC曲线下面积分别为0.693、0.682、0.684,诊断效能最好的指标为CD3^(+)绝对计数。Spearman相关性分析结果显示,55例髋关节一过性滑膜炎患儿外周血CD3^(+)、CD4^(+)、CD8^(+)绝对计数与关节囊前壁厚径存在负相关关系(rs值分别为-0.408、-0.373、-0.366,P<0.05),负相关性最好的指标为CD3^(+)绝对计数。结论:临床上T淋巴细胞亚群中CD3^(+)、CD4^(+)、CD8^(+)及CD4^(+)/CD8^(+)的表达水平对儿童髋关节一过性滑膜炎的评估及预后,以及有针对性地进行免疫治疗具有一定的指导意义。OBJECTIVE To detect the expression levels of T lymphocyte subsets CD3^(+),CD4^(+),CD8^(+)and CD4^(+)/CD8^(+)in peripher⁃al blood of children with transient hip synovitis,and to explore its clinical significance in the diagnosis,treatment and severity of the disease.METHODS A total of 130 children with TSH admitted to Changchun Children’s Hospital from April 2019 to December 2021 were selected as the observation group,and 30 normal children who underwent physical examination in the hospital during the same period were selected as the control group.The number of T lymphocyte subsets in peripheral blood of the two groups was analyzed,and receiver operating characteristic curve(ROC curve)was drawn with P<0.05 detection index to analyze its diagnostic efficiency.The correlation between T lymphocyte subsets in peripheral blood and the thickness of anterior capsular wall and the depth of joint effusion was analyzed in 55 children with positive hip color ultrasound results.RESULTS The absolute count of total T lymphocytes CD3^(+),the absolute count of adjuvant/induced T lymphocytes CD4^(+),and the absolute count of inhibitory/killer T lym⁃phocytes CD8^(+)in the observation group were significantly lower than those in the control group,and the differences were statistical⁃ly significant(Z=-3.296,-3.097,-3.139,P<0.05).The area under ROC curve of absolute counts of CD3^(+),CD4^(+)and CD8^(+)in the observation group were 0.693,0.682 and 0.684,respectively,and the absolute count of CD3^(+)was the best indicator of diag⁃nostic efficiency.Spearman correlation analysis showed that absolute counts of CD3^(+),CD4^(+),CD8^(+)in peripheral blood were negative⁃ly correlated with anterior capsular wall thickness in 55 patients with transient hip synovitis(rs=-0.408,-0.373,-0.366,P<0.05).CONCLUSION The expression levels of CD3^(+),CD4^(+),CD8^(+)and CD4^(+)/CD8^(+)in T lymphocyte subsets have certain guiding signifi⁃cance for the evaluation and prognosis of transient hip synovitis in children,as well as targeted immunothe
