假白喉棒状杆菌引起皮肤软组织感染1例

摘　　要：目的通过对临床分离的假白喉棒状杆菌及其引起的皮肤软组织感染进行分析,为假白喉棒状杆菌的临床感染诊治提供依据。方法采集患者压疮深部分泌物和组织,进行革兰染色和细菌培养,对分离到的病原菌利用DL-96Ⅱ细菌测定系统和基质辅助激光解析电离飞行时间质谱仪进行菌种鉴定。同时利用120Coryne棒状杆菌药敏卡对病原菌进行药物敏感性测定。结果分泌物和组织涂片染色可见革兰阳性杆菌,需氧培养出革兰阳性杆菌,经鉴定为假白喉棒状杆菌。药敏试验显示红霉素、环丙沙星和克林霉素耐药,万古霉素、利奈唑胺、青霉素、三代(四代)头孢菌素及庆大霉素等敏感。结论本病例强调了假白喉棒状杆菌是皮肤软组织感染的一种条件致病菌,实验室和临床应重视该细菌引起的感染,抗微生物治疗是必要的。Objective By analyzing clinical isolates of Corynebacterium pseudodiphtheriae and the soft-tissue skin infections they cause,to provide a basis for the diagnosis and treatment of clinical infections with Corynebacterium pseudodiphtheriae.Methods Collect the secretions and tissues of the patients with pressure sore,gram staining and bacterial culture,the isolated pathogens were identified by DL-96Ⅱbacterial detection system and matrix-assisted laser desorption/ionization time-of-flight m ass spectrometer.At the same time,the drug sensitivity of pathogens was determined using 120Coryne Corynebacterium drug sensitivity card.Results Gram-positive bacilli seen in secretions and tissue smear stains,aerobic culture of gram-positive bacilli,identified as Corynebacterium pseudodiphtheria.Drug sensitivity tests showed resistance to erythromycin,ciprofloxacin and clindamycin,and sensitivity to vancomycin,linezolid,penicillin,third(fourth)generation cephalosporins,and gentamicin.Conclusion This case emphasizes that Corynebacterium pseudodiphtheria is a conditional pathogen of skin and soft tissue infection.laboratory and clinical should pay attention to the infection caused by this bacteria,antimicrobial therapy is necessary.
