摘 要:随着近几年抗菌药物的广泛应用,多重耐药鲍曼不动杆菌检出率明显增加,且在多地出现过爆发。2020年,世界卫生组织将多重耐药鲍曼不动杆菌归类为迫切需要开发抗菌药物的“最优先病原体”。随着对多重耐药鲍曼不动杆菌的研究,发现其耐药机制主要有五类,分别是β-内酰胺酶的产生、主动外排泵的过度表达、外膜通透性改变、氨基糖苷类药物修饰酶、药物作用靶位改变。在多重耐药鲍曼不动杆菌泛滥的时代,其耐药机制的明确为开展和研发新的治疗方法及药物提供了方向。本文主要从多耐药鲍曼不动杆菌耐药机制、治疗药物、治疗方式以及新型治疗研究进行综述。With the widespread use of antibiotics in recent years,the detection rate of multidrug-resistant A.baumannii has increased significantly and outbreaks have occurred in many places 2020,the World Health Organization classified multidrug-resistant A.baumannii as a“top priority pathogen”for which the development of antibiotics is urgently needed.With the study of multidrug-resistant A.baumannii,five main resistance mechanisms have been identified,which are carbapenemase production,overexpression of active efflux pumps,alteration of outer membrane permeability,aminoglycoside-modifying enzymes,and alteration of the target site of drug action.In the era of the proliferation of multidrug-resistant A.baumannii,the clarification of its resistance mechanism provides a direction to carry out and develop new therapeutic approaches and drugs.In this paper,we review the drug resistance mechanism,therapeutic drugs,treatment modalities,and therapeutic research directions of multidrug-resistant A.baumannii.
