椎间孔镜BEIS与TESSYS技术治疗腰椎间盘突出症患者的疗效比较

摘　　要：目的 探讨椎间孔镜宽视野简易即刻手术(BEIS)技术与经皮椎间孔入路椎间孔镜系统(TESSYS)技术对腰椎间盘突出症(LDH)患者疼痛和腰椎功能的影响。方法 回顾性分析2017年8月至2019年12月该院收治的LDH患者400例,根据不同治疗方式分为观察组和对照组,各200例,对照组采用TESSYS技术进行治疗,观察组采用BEIS技术进行治疗。比较2组手术相关指标及治疗有效率;比较2组术前、术后2个月Oswestry功能障碍量表(ODI)评分、视觉模拟量表(VAS)评分、腰椎曲线指数、腰椎矢状位Cobb角、椎间隙高度,以及并发症发生率和复发率。结果 观察组的手术时间、术后卧床时间短于对照组,术中出血量少于对照组,切口长度短于对照组,差异均有统计学意义(P<0.05)。观察组VAS评分、ODI评分均低于对照组(P<0.05)。观察组治疗有效率、腰椎曲线指数、腰椎矢状位Cobb角及椎间隙高度均高于对照组(P<0.05)。观察组并发症发生率低于对照组,但差异无统计学意义(P>0.05),复发率低于对照组(P<0.05)。结论 BEIS技术能够有效改善LDH患者腰椎功能,有助于缓解腰椎疼痛,提升治疗有效率,值得临床推广应用。Objective To explore the effects of broad easy immediate surgery(BEIS)and transforaminal endoscopic spine system(TESSYS)on pain and lumbar spine function in the patients with lumbar disc herniation(LDH).Methods A retrospective analysis was conducted on 400 patients with LDH admitted and treated in this hospital from August 2017 to December 2019.The patients were divided into the observation group and control group according to different treatment methods,200 cases in each group.The control group adopted the TESSYS technique,while the observation group adopted the BEIS technique.The surgical related indicators and treatment effective rate were compared between the two groups;the Oswestry functional index(ODI)scores,Visual Analogue Scale(VAS)scores and lumbar spine curve index,lumbar spine sagittal Cobb angle and intervertebral space height as well as incidence rate of complications and recurrence rates in the two groups were compared between before operation and in postoperative 2 months.Results The operation time and postoperative time in bed in the observation group were shorter than those in the control group,the intra operative bleeding volume was less than that in the control group,the incision length was shorter than that in the control group,and the differences were statistically significant(P<0.05).The VAS scores and ODI score in the observation group were lower than those in the control group(P<0.05);the treatment effective rate,lumbar spine curve index,lumbar spine sagittal Cobb angle and intervertebral space height in the observation group were higher than those in the control group(P<0.05);the incidence rate of complications in the observation group was lower than that in the control group,but the difference was not statistically significant(P>0.05).The recurrence rate in the observation group was lower than that in the control group(P<0.05).Conclusion The BEIS technology could effectively improve the lumbar function of LDH patients,help to alleviate lumbar pain and improve treatment efficiency,which i
